不同时期小儿上消化道疾病胃镜对比分析

摘　　要：目的探讨不同时期小儿上消化道疾病胃镜检出率及幽门螺旋杆菌(Hp)感染情况,为临床诊治提供科学依据。方法我院于2005年、2009年分别收治有消化道症状的患儿68例和133例,对所有患儿进行电子胃镜检查并取胃黏膜行尿素酶试验、病理学检测Hp,对两年检查结果进行对比分析。结果 2005年胃镜对疾病总体检出率为91.18%,2009年为95.49%;2005年Hp检出率为16.18%,2009年为18.80%。两年胃镜检查病变总检出率、Hp检出率差异无显著性(P>0.05),但2009年慢性浅表性胃炎、十二指肠球炎检出率分别为52.63%、26.32%,均高于2005年(36.76%,13.24%)(P<0.05)。结论近4a我地区小儿胃Hp检出率无明显下降,慢性浅表性胃炎、十二指肠球炎有增多趋势,提示应进一步加强小儿上消化道疾病及Hp感染的防治工作。Objective To explore the morbidities of upper alimentary disease and of Helicobacter pylori（Hp） infection in children at different periods inorder to improve guideline to diagnosis and treatment.Methods Our hospital in 2005,2009,admitted there were 68 cases of children with gastrointestinal symptoms and 133 cases of children of all electronic endoscopy and gastric mucosa urease test line,pathology detection of Hp,on the two-year inspection The results were compared.Results 91.18%（62/68） cases had lesions in 2005 and 95.49%（127/133） in 2009.The positivity rate of Hp in 2005 was 16.18%（11/68） and 18.80%（25/133） in 2009.There was not signif icant difference in the overall detectable rate of upper alimentary diseases and Hp infection between 2005 and 2009（P0.05）.Among all the upper alimentary diseases,the incidence of chronic superf icial gastritis,duodenitis in 2009 was 52.63%,26.32%,respectively,wh ich were higher than those in 2005（P0.05）.Conclusion Compare with 4 years ago,the incidence of chronic superf icial gastritis,duodenitis were increased in our region,and the rate of Hp infection in children was not changed signif icantly.This inicate the system of the interference and prevention of children？s upper alimentary disease and Hp infection are incomplete,which need us take measures to improve it.
