708例护理不良事件分析及法律风险初探

摘　　要：目的通过分析708例护理不良事件,探讨主要护理不良事件存在的法律风险大小及关系。方法对某三级甲等医院2007～2009年护理系统上报的708例护理不良事件进行分类统计,对其中存在法律风险的不良事件进行统计学处理,评估分析其法律风险大小、关系。结果护理不良事件中法律风险最大的系统为精神科系统;法律风险最大的患者损伤级别为Ⅱ级;法律风险最大的不良事件类型为用药错误、药敏差错、实习生单独操作错误。结论要降低护理不良事件的法律风险,不仅要降低不良事件发生率,还要根据不同系统、级别、类型不良事件各自的法律风险大小有针对性的配置资源、完善流程、引进管理系统和进行人员培训。Objective To investigate the legal risk of adverse events in nursing,the 708 cases were analyzed.Methods 708 cases of adverse events,reported by nursing department of a three-graded-class A hospital during 2007-2009,were classified,and their legal risk was analyzed.Legal risk of every classification was assessed,and explored the relathionship between classification and legal risk of the adverse events.Results Of adverse events in nursing,the highest frequency was in surgical deparment,the most at risk was in psychiatric deparment.Of adverse events in nursing,the highest frequency of injury was gradeⅠ,most at legal risk was gradeⅡ,the most serious legal consequences was gradeⅤ,Ⅵ.Of adverse events in nursing,the highest frequency classifications were medication errors,treatment errors and nursing errors;the most at risk of legal classifications were medication errors,errors in drug allergy and intern separate operation error;the most serious legal consequences classif ications were wounding,self-injury,suicide,out without permission.Conclusion To redue legal risks of adverse events in nursing,not only its incidence was reduced;but also according to different departmen,grade,classification,and their legal risk,the resources were targeted allocated,processes were improved,management systems were introductied,staff was trained.
