电子病历“有效使用”的概念及对推广应用的意义

摘　　要：美国经济复苏和投资刺激计划(American Recovery and Reinvestment Act,ARRA)拨出了一大笔预算(第一年150亿美元)用于电子健康病案(EHR)的推广应用。美国国家医疗服务保险机构CMS(the Centers for Medicare & MedicaidServices)向国会提交了一份建议书,提出一系列指标用于评价医疗卫生机构是否"有效使用"(Meaningful Use)了电子病案系统。CMS提出23条评判医院"有效使用"EHR的准则,从2011年开始,凡使用电子病历并达到这23条要求的机构,就可得到ARRA法案的财政补贴,到201 5年还达不到要求,则要接受罚款惩罚。美国医院协会作了一个调查,提供了一个报告叫TrendWatch Report。结论是:尽管医疗卫生机构都急切的想使用电子病案系统,但到2015年,该法案的惩罚条款生效时,55%的医疗机构会因为达不到CMS所定的条件,受到惩罚。American Recovery and Reinvestment Act （ARRA） allocated a large sum of budget （S15 billion in the first year） to spread and apply the electronic health record （EHR）. The Centers for Medicare ＆ Medicaid Services （CMS） submitted a proposal book to the Parliament, and put forward a series indexes to estimate if the medical health institutions make the ＂Meaningful Use＂ of the electronic medical record system. CMS brings forward 23 rules to judge if hospital makes the ＂Meaningful Use＂ , and since 2011, the medical institution can obtain the financial subsidy provided by ARRA act, which makes use of the electronic medical record and satisfies 23 rules, or until 2015, will be imposed a fine. American Hospital Association conducted a survey and provided a report called TrendWatch Report, which Conclusion is： Although the medical institutions are all eager to use electronic medical records system, 55% of medical institutions were punished because they failed to the conditions set by CMS by 2015, where the penalty provisions of this bill come into force.
