血液透析与血液透析滤过对低血压影响的临床比较

摘　　要：目的探讨血液透析(HD)和血液透析滤过(HDF)在透析治疗过程中低血压的发生情况。方法 56例稳定性血液透析患者随机分成3组,行HD、前稀释HDF、后稀释HDF治疗,观察治疗前、治疗中每小时治疗末时血压及低血压的次数。结果前稀释HDF组、后稀释HDF组低血压的发生率低于HD组;前稀释HDF组在治疗末的平均动脉血压(MAP)高于后稀释HDF组。结论血液透析滤过可降低透析过程中低血压的发生率,前稀释血液透析效果优于后稀释血液透析滤过。Objective To investigate the incidence of hypotension in the hemodialysis（HD）and hemodiafiltration（HDF）in the dialysis treatment.Methods 56 patients with stable hemodialysis patients were randomly divided into three groups,line HD,pre-dilution HDF,post-dilution HDF treatment,blood pressure and the times of low blood pressure before treatment in hourly and at the end of treatment were observed.Results Pre-dilution HDF group,post-dilution HDF group the incidence of hypotension than HD group;mean arterial blood pressure（MAP）of pre-dilution HDF group therapy at the end of treatment were higher than the post-dilution HDF group.Conclusion Hemodiafiltration during dialysis can reduce the incidence of hypotension,pre-dilution effect is better than post-dilution hemodiafiltration.
