摘  要:目的探讨低能量无骨折脱位性颈脊髓损伤的病例特点及治疗方法。方法 30例由微小外力致颈脊髓损伤的一组病例,根据影像学特点和脊髓损伤程度分类,选择性治疗,观察近期疗效。结果①Ⅰ型:20%患者椎管狭窄,无明显脊髓受压,脊髓损伤轻,保守治疗疗效满意。Ⅱ型:60%患者椎管狭窄并间盘突出等致脊髓受压。Ⅲ型:20%患者颈椎不稳并间盘突出。Ⅱ、Ⅲ型适合手术治疗,疗效显著。②前路疗效优于后路。③1周内和1-3周手术疗效无差异。结论低能量无骨折脱位性颈脊髓损伤应根据患者临床特点选择合适的治疗方法。【Objective】To investigate the feature and treatment of cervical spinal cord injuries without fracture or dislocation caused by lower energy.【Methods】30 cases of cervical spinal cord injury without fracture or dislocation caused by lower energy were studied. Different treatments were performed according to their imaging features and the degree of injury and their short-term outcomes were evaluated. 【Results】①Ⅰ: About 20% patients suffered from light spinal cord injury without spinal compression and expectant treatment was effective. Ⅱ: About 60% pa- tients suffered from spinal compression caused by disc protrusion with decrease of the storing space of spinal canal. Ⅲ: About 20% patients suffered from segmental instability among the vertebras combined with disc herniation. Ⅱ, Ⅲ were both fit for operation, which therapeutic effect was good. ② Anterior approach was better than the posterior. ③ The result of operation had no difference between 1 week's and 1-3 week’s groups.【Conclusions】The treatment of cervical spinal cord injuries without fracture or dislocation caused by lower energy should be performed according to the features of patients
