摘 要:目的探讨64层螺旋CT(64-SCT)与经胸超声心动图(TTE)两种检查方法在先心病诊断中的价值。方法收集2007年9月~2009年3月在本院经手术证实的先心病病例54例,所有患者术前均同时行64-SCT和TTE检查,以手术为金标准,评价两种检查方法的诊断准确率。结果 54例共证实畸形176处,64-SCT诊断准确率为98.3%,TTE诊断准确率为90.3%。TTE漏误诊主要为心外大血管畸形(15/18)。在诊断心内畸形及心脏-大血管连接畸形方面两者无显著性差异。结论 64-SCT和超声心动图在先心病诊断中各有优势和不足,联合应用两种检查方法有助于提高复杂先心病的诊断准确率。Objective To discuss the value of 64-slice spiral CT(64-SCT) and transthoracic echocardiography(TTE) in the diagnosis of congenital heart diseases(CHD).Methods Fifty-four cases of congenital heart disease which were proved by surgery in our hospital from September 2007 to March 2009 were collected.All of these patients underwent 64-SCT and TTE at the same time before operation.Then,the operation results were taken as diagnostic standard to evaluate the diagnostic accuracy of these two methods.Results In the 54 cases 176 malformation were confirmed.The diagnostic accuracy of 64-SCT was 98.3%,and TTE was 90.3%.The misdiagnosis of TTE mainly was major extracardiac malformations(15/18).In the diagnosis of cardiac malformation and heart-large vascular connecting malformation,the two methods had no significant difference.Conclusion 64-SCT and echocardiography have their own advantages and shortcomings in the diagnosis of heart disease.Joint application of the two methods helps to improve the diagnostic accuracy of complex CHD.
