普外科危重患者营养状态改变及其对免疫功能的影响

摘　　要：普外科病人危重组入院48h内与对照组相比，躯体测量、白蛋白变化不大，而视黄醇结合蛋白等短半衰期血清蛋白质指标明显低下，免疫功能指标严重受损；7天后营养指标全部下降，免疫功能指标进一步损害。说明，躯体测量实验室检查能全面地判断普外危重患者营养状态，视黄醇结合蛋白是评定营养状态最敏感的血清蛋白质指标。The aim of this sutdy was to evaluate the variation of nutritional state and immnune function in critical cases of general surgery. Thirty cases were divided into two groups. Fifteen critical cases were included as critical group while fifteemlight cases as control group. All parameters in two groups wre compared which were Ba TSF AC AMC ALB TF PA RBPFN TLC T cell subset ig IL - 6 and SIL - 2R. The results showed that there were not significant variance of anthropomet.ric measurements and ALB between two groups within 48 hours after admission, but those short half -- life protein indicesincluded RBP and immune indices decreased significantly. Seven days later, the nutritional and immune parameters continuously decreased in critical group. It can be concluded that the corn-bination of anthropometry and laboratory exanunation is more advantageous than each other while evaluating nutritional state and immune function in critical cases of general surgery, RBP is the most sensitive protein index in nutritional evaluation at present.
