摘  要:目的 探讨HBO治疗急性脑梗死后抑郁症(post-stroke depression,PSD)的临床效果及机制.方法 78例PSD患者分为治疗组和对照组各39例,对照组行常规治疗,治疗组在此基础上加用HBO治疗.评估2组治疗前后汉密尔顿抑郁量表(HAMD)评分、神经功能缺损评分(采用美国国立卫生研究院卒中量表,NIHSS),并用经颅多普勒(TCD)监测治疗前后脑血流量.结果 2组的HAMD评分和NIHSS评分治疗后均较治疗前明显改善(P〈0.01).治疗组改善更显著,脑血流量治疗后较治疗前明显增加(P〈0.01或P〈0.05).对照组治疗前后差异无统计学意义(P〉0.05).结论 HBO治疗PSD疗效显著,并能改善抑郁症所导致的脑血流变化.Objective To investigate the effect of hyperbaric oxygen (HBO) on post-stroke depression (PSD) and its possible mechanism involved.Methods Seventy-eight cases of PSD were randomly divided into 2 groups:the therapeutic group and the control group,each group consisting of 39 cases.The therapeutic group was treated with HBO in addition to routine medication.Melancholia was assessed with the Hamilton Depression Scale (HAMD).Neural deficiency Was assessed with the NIH Stroke Scale (NIHSS).Changes in the blood flow of anterior cerebral artery (ACA),midge cerebral artery (MCA) and posterior cerebral artery (PCA) were detected with TCD.Results Scores obtained from HAMD and NIHSS showed that there was significant improvement in both groups after treatment (P〈0.01),when they were compared with those before treatment, with the HBO group having more significant improvement.Furthermore,cerebral blood flow for the therapeutic group after treatment increased more significantly than before treatment (P〈0.01 or P〈0.05),while no differences could be seen in the control group both before and after treatment (P〈0.05) with statistical significance.Conclusions HBO proved to be effective for the treatment of PSD and could improve cerebral blood flow induced by depression.
