主动脉脉压对原发性高血压患者肌酐清除率的影响

摘　　要：目的:探讨原发性高血压患者有创主动脉脉压(PP)与肌酐清除率(Ccr)的关系。方法:将入选的365例原发性高血压患者按PP分为2组,PP≤60mmHg(1mmHg=0.133kPa)180例(A组),PP>60mmHg185例(B组),在冠状动脉造影术中测量主动脉血压。测定患者的体质量、空腹血糖、血脂、血尿素氮、尿酸和肌酐等指标,通过Cockcroft-Gault方程计算Ccr,对主动脉脉压与Ccr进行偏相关分析。结果:(1)A组年龄(60.29±9.13)岁,收缩压(122.37±16.69)mmHg,脉压(47.17±10.75)mmHg,B组年龄(61.62±9.17)岁,收缩压(157.05±20.12)mmHg,脉压(82.47±15.27)mmHg,以上几个指标B组均高于A组(P<0.05或P<0.01),而B组Ccr为(81.34±30.33)mL/min,低于A组的(98.38±30.82)mL/min,差别有统计学意义(t=2.199,P<0.01)。(2)去除术中收缩压的影响,脉压与肌酐清除率的偏相关系数为-0.234(P<0.001),年龄与肌酐清除率的偏相关系数为-0.676(P<0.001),年龄与脉压的偏相关系数为0.237(P<0.001)。去除收缩压和年龄因素的影响,脉压与肌酐清除率的偏相关系数为-0.104(P=0.045)。结论:利用有创主动脉脉压测定预测患者的肾功能损害价值有限。Objective：To explore the relationship between aortic pulse pressure（PP）and creatinine clearance rate（Ccr） in patients with essential hypertension.Methods：Three hundred sixty-five patients with essential hypertension were enrolled and underwent coronary angiography.The aortic blood pressure was measured during angiography.Patients were divided into group A（PP≤60 mm Hg） and group B（PP〉 60 mm Hg）.Indexes such as age,body weight,glucose,lipids,blood urea nitrogen,uric acid and creatinine were measured.The level of Ccr was calculated by Cockcroft-Gault equations,and the relationship was analyzed between PP and Ccr was analyzed by partial correlation analysis.Results：In group B the age（61.62±9.17） years old,systolic blood pressure（157.05±20.12） mm Hg,pulse pressure（47.17±10.75） mm Hg were significantly higher than those of group A.However,the level of Ccr was significantly lower in group B（81.34 ±30.33） mL/min than that of group A（98.38±30.82） mL/min（t = 2.199,P 〈0.01）.The positive correlation was gained between age and aortic pulse pressure（r = 0.237,P〈 0.001）.The negative correlation was proved between aortic pulse pressure and Ccr（r =-0.234,P 〈0.001）.Aortic pulse pressure was lower and negatively correlated with Ccr after deducing influence of age factors（r =-0.104,P = 0.045）.Conclusion：There was a limited value to determine the renal dysfunction by aortic blood pressure.
