摘 要:目的探讨气囊仿生助产术的有效性和安全性。方法将符合相关条件的2410例产妇随机分为观察组和对照组。观察组除临产后在宫口扩张2~4cm时行双扩法气囊仿生助产术外,其他产科处理措施与对照组相同。结果观察组第一产程、第二产程和总产程短于对照组(P<0.01、P<0.05、P<0.05),自然分娩率高于对照组(P<0.01),产钳助娩率和剖宫产率低于对照组(P<0.01、P<0.05),产后2h内阴道出血量和产后出血率低于对照组(均P<0.05),胎儿窘迫率和缩宫素催产率低于对照组(均P<0.01);两组新生儿窒息率、软产道损伤率、阴道分娩后24h血白细胞计数、中性粒细胞比例、血清C-反应蛋白(CRP)和白介素-6(IL-6)水平、产后尿潴留发生率差异均无统计学意义(均P>0.05)。结论气囊仿生助产术能显著缩短产程,提高阴道自然分娩率,降低手术产率,减少产后出血量,降低胎儿窘迫率和缩宫素催产率,是一种简便、有效、安全、适宜的现代助产技术,值得应用和推广。Objective To investigate the efficacy and safety of bionic air-bag midwifery skill.Methods Totally 2410 primiparas were randomly divided into air-bag group and control group.Women in air-bag group ware given the same obstetrical treatments as that in the control group except bionic air-bag midwifery skill was used twice when the cervix was dilated 2~4 cm during labor.Results The time of first stage,second stage and total delivery process in the air-bag group were shorter than those in control group(P0.01,P0.05,P0.05).The natural delivery rate in the air-bag group was higher than that in the control one.The forceps assited delivery rate and the cesarean section rate in air-bag group were lower than those in control group(P0.01,P0.01,P0.05).The amount of vaginal bleeding within two hours after delivery and the rate of postpartum hemorrhage were less than those in the control group(P0.05,P0.05).The rate of fetal distress and rate of using oxytocin in the air-bag group were lower than those in control group(P0.01,P0.01).The rate of maternal and fetal complications such as neonatal asphyxia and soft labor canal injury had no significant differences in the two groups.The bionic air-bag midwifery skill had no contribution to the outcome of incidence rate of urine retention,similar results were found in leukocyte count,neutrophil proportion and the level of c-reactive protein(CRP)and interleukin-6(IL-6)24 hours after delivery.Conclusions The bionic air-bag midwifery skill is a simple,effective and safe procedure,which is worthy of popularizing and application.
