HST-302血细胞分析流水线有关复检规则的建立

摘　　要：目的制定适合笔者所在实验室HST-302血细胞分析流水线常规工作需求的自动血细胞计数与WBC分类的复检规则。方法采用日本希森美康公司生产的HST-302血细胞分析流水线,随机检测患者标本1212份,同时涂片镜检,观察细胞形态并进行人工WBC分类。与国内"XE-2100复检协作组"制定的"XE-2100复检规则23条"进行应用评估,以镜检结果为金标准,统计真阳性、假阳性、真阴性、假阴性比率,筛选出符合笔者所在实验室常规工作需求的血细胞复检规则。结果统计结果显示真阳性率47.5%(576/1212),假阳性率14.6%(177/1212),真阴性率29.0%(351/1212),假阴性率8.9%(108/1212);假阳性出现较多的规则是嗜碱性粒细胞绝对值、未成熟粒细胞、核左移、原始细胞和有核红细胞,假阴性出现最多的规则是未成熟粒细胞及核左移。根据以上统计结果 ,最终制定出符合笔者所在实验室特点和要求的血细胞分析和WBC分类复检规则,该规则的复检率为26.5%(53/200),真阳性率为44.5%(89/200),假阳性率为19.0%(38/200),真阴性率为33.0%(66/200),假阴性率为3.5%(7/200)。结论制定出的复检规则适合笔者所在实验室的临床工作需求,且具有一定推广意义。Objective To establish the suitable hematology review criteria in Sysmex HST-302 hematology analyzer assembly line for the routine work of laboratory. Methods The total 1212 patient specimen were randomly detected by Sysmex HST-302 hematology analyzer assembly line and blood smear was reviewed including observation of cell morphology and manual WBC differential analysis. Base on the 23 items of hematology review criteria suggested by internal XE-2100 cooperation group, the true positive ratio （TP）, the false positive ratio（FP）, the true negative ratio （TN）, and the false negative ratio （FN） were evaluated. Then the suitable review criteria for the routine work of laboratory were screened. Results TP,FP,TN and the FN were 47.5%（576/1212）,14.6%（177/ 1212）,29.0%（351/1212）and 8.9% （108/1212）,respectively. ＂Baso#＂,＂Immature Gran＂,＂Left shift flag＂,＂Blast＂and ＂NRBC＂were the most frequent FP flag,＂Immature Gran？＂ and ＂Left shift flag＂were the most frequent FN flag. According to the above-mentioned results and clinicians opinions the suitable criteria for author＇s laboratory. In the established criteria the review ratio was 26.5%（53/200）, TP was 44.5% （89/200）, FP was 19.0%（38/200）,TN was 33.0%（66/200）,FN was 3.5%（7/200）. Conclusion The hematology review criteria is suitable for the routine work of author＇s laboratory, and has certain populazation significance.
