何氏正骨手法治疗三踝骨折的临床研究

摘　　要：目的:本课题是在继承何氏正骨手法的基础上,观察和总结何氏正骨手法治疗三踝骨折的临床疗效。方法:共搜集100例病例,随机分为治疗组50例,对照组50例。治疗组为中医跟骨牵引、正骨手法并小夹板外固定进行治疗;对照组为西医手术切开复位内固定并石膏外固定进行治疗。所有病例根据Baird踝关节评分标准分别在治疗后第2天、2周、4周、8周、24周进行评分并比较。结果:治疗组和对照组在治愈率及有效率上比较差异无统计学意义;治疗后第2天、第2周、第4周、第8周,治疗组和对照组在症状缓解上比较差异有统计学意义;而治疗后第24周,治疗组和对照组在症状缓解上比较差异无统计学意义。结论:何氏正骨手法可以有效的治疗三踝骨折这种复杂性骨折,并且随着治疗时间的推移,患者症状可以得到逐步改善。Objective：To summarize he the experience of He＇s bonesetting in the treatment of trimalleolar fracture. Methods：Of the 100 cases with trimalleolar fracture, 50 in the treatment group were treated by calcaneus traction, bonesetting plus splint external fixation, while the other 50 in the control group were treated by open reduction plus plaster external fixation. After 2 days, 2 weeks, 4 weeks, 8 weeks and 24 weeks, we used Baird Scoring System for Ankle Fracture to assess the ankle function in all cases. And the clinical effective rate was evaluated after 8 weeks treatment. Results：There were no significant differences in cure rate and total effective rate between treatment group and control group. There was significant difference in improvement of the patients＇ symptoms between the two groups after being treated for 2 days, 2 weeks, 4 weeks and 8 weeks. But after 24 weeks, there was no significant difference. Conclusion：The He＇s bonesetting is effective for trimalleolar fracture, and the patients＇ symptoms can be gradually improved.
