摘  要:目的探讨ω-3鱼油脂肪乳对全身炎症反应综合征(SIRS)患者血清C-反应蛋白和体液免疫功能的影响。方法选择2009年2月~2010年1月入住我院普外科的全身炎症反应综合征患者45例,随机分为全胃肠外营养组(A组)25例和鱼油+TPN组(B组)20例,两组患者均接受等氮等热量的TPN,B组每人每天加用ω-3鱼油脂肪乳100mL,共5d,分别于应用TPN前和应用TPN5d后检测血清C-反应蛋白和免疫球蛋白(IgA、IgG、IgM)浓度。结果治疗5d后,C-反应蛋白、APACHEⅡ评分两组比较具有统计学意义(P<0.05);免疫球蛋白B组在治疗后无明显变化,A组在治疗后IgA、IgG显著下降,两组治疗后比较具有统计学意义(P<0.05)。结论ω-3鱼油脂肪乳可以明显降低SIRS患者C反应蛋白水平,减轻炎症反应;同时可以增强SIRS患者体液免疫功能。Objective To explore the effects of ω-3 fish oil fat emulsion on the C-reactive protein (CRP) and humoral immune function in patients with systemic inflammatory response syndrome(SIRS). Methods Forty-five patients with SIRS were randomly divided into routine total parenteral nutrition (TPN) group (Group A,25 cases) and fish oil fat emulsion + TPN group (Group B, 20 cases),and both groups received total parenteral nutrition with equal nitrogen content and calories, and the ω-3 fish oil fat emulsion 100mL/p·d was added to Group B, lasting 5days, The CRP level and immune globulin were checked respectively before giving TPN, and on Day 5 after giving TPN. Results After 5-day treatment, there was a significant difference in CRP and APACHE Ⅱ between the two groups (P〈0.05). There was no significant change in immune globulin in Group B after treatment and,IgA and IgG were significantly decreased in Group A after treatment,with a significant difference between the two groups (P〈0.05). Conclusion The ω-3 fish oil fat emulsion can decrease significantly the CRP level, reduce inflammatory reaction and enhance humoral immune function in the patients with SIRS.
