彩色多普勒超声在睾丸扭转中的诊断价值

摘　　要：目的探讨彩色多普勒超声在睾丸扭转中的诊断价值。方法回顾分析我院4年来通过彩超诊断和手术治疗的15例睾丸扭转患者的超声图像改变及临床诊治结果。结果睾丸扭转超声图像改变和扭转时间及睾丸扭转程度密切相关,表现为睾丸增大或缩小、回声不均匀、弥漫性偏低回声或片状高回声、精索增粗、阴囊壁水肿等,特别是彩色多普勒超声的应用,大大增加了睾丸扭转检出率,本组实验检出率约93%,表现为血流减少或流速减低及血流信号消失。结论彩色多普勒超声在睾丸扭转的诊断及鉴别诊断中具有极其重要的价值,对临床诊治具有指导意义。Objective To evaluate color Doppler flow imaging（CDFI） in the diagnosis of testicular torsion. Methods Fifteen cases of testicular torsion were reviewed in our hospital for four years and their specific uhraphonic images and clinical results were analyzed. Results The ultraphonic images of testicular torsion were closely correlated with the time and the degree of torsion. The specific appearance of testicular torsion showed an increased or decreased testicle,asymmetrical echo,diffusive low echo or flaky high echo, thickening varicosity,turgescent scrotum wall,etc. The detection rate of testicular torsion was significantly increased by using CDFI,with the detection rate of 93%, showing decreased blood flow or blood flow velocity and disappearing blood flow signal. Conclusion CDFI has a great value in the diagnosis of testicular torsion, with a guide value in its clinical diagnosis and treatment.
