溃疡性结肠炎中医辨证分型与凋亡调节因子关系的研究

摘　　要：目的检测细胞凋亡调控蛋白Bcl-2/Bax在溃疡性结肠炎(UC)患者结肠黏膜组织上皮细胞的表达,探讨Bcl-2/Bax蛋白在UC发病机制中的作用及与中医证型的相关性。方法溃疡性结肠炎患者(观察组)20例经中医辨证为大肠湿热型组(10例),脾胃虚弱型组(10例);用免疫组织化学方法检测Bcl-2/Bax蛋白在结肠黏膜组织上皮细胞内表达情况,并与正常对照组(10例)进行比较。结果大肠湿热型组肠黏膜腺上皮细胞中Bax蛋白表达最高,与正常组和脾胃虚弱型组比较均有显著性意义(P<0.05);与正常对照组比,大肠湿热型组Bcl-2蛋白表达明显降低(P<0.05),脾胃虚弱型组Bcl-2蛋白表达无明显差异(P>0.05);大肠湿热型组Bcl-2蛋白与脾胃虚弱型组间比较具有显著差异(P<0.05)。大肠湿热型组Bcl-2蛋白表达降低。结论细胞凋亡调控蛋白Bcl-2/Bax表达与UC发生发展和中医证型相关,其表达水平可作为UC中医辨证分型新的诊断依据;对协助UC中医辨证分型及确定治疗方案具有重要作用。Objective To study the correlation between apoptosis regulatory proteins Bcl-2 and Bax on the intestinal mucosa in patients of ulcerative colitics and TCM differentiation of syndromes typing.Methods 20 patients with UC were divided into 2 groups.According to TCM differentiation of syndroms including dachangshire syndrome group（10 cases）,piweixuruo syndrome group（10 cases）.The expression of Bcl-2,Bax in two syndromes were detected and compared by immunomistochemistry.Results The expressions of Bax were highest in dachangshire syndrome group.There were significant differences between the piweixuruo syndrome group and normal control group（P0.05）.Controling with normal control group,the expression of Bcl-2 in dachangshire syndrome group were significantly decreased（P0.05）,but there were no statistical difference in piweixuruo syndrome group（P0.05）.The expressions of Bcl-2 in dachangshire syndrome group were significantly lower than those in the piweixuruo syndrome group（P0.05）.Conclusion The expressions of Bcl-2 and Bax are related to the pathogenesis of UC and TCM typing.The expressions of Bcl-2 and Bax in UC patients are helpful to the TCM typing according to differentiation of syndromes and to selection of trearment plans.
