丹参注射液介入再通治疗输卵管阻塞性不孕的量效关系初步研究

摘　　要：目的观察丹参注射液在介入再通灌注术治疗输卵管阻塞性不孕症的临床疗效及量效关系。方法 200例输卵管阻塞性不孕患者,采用输卵管介入再通术治疗闭塞输卵管,再通成功后试验组分别在输卵管腔内灌注不同剂量的丹参注射液(A组:丹参注射液6 mL、生理盐水14 mL;B组:丹参注射液10 mL、生理盐水10mL;C组丹参注射液14 mL、生理盐水6 mL),对照组(D组)注入庆大霉素、地塞米松、α-糜蛋白酶及生理盐水。试验组术后采用中医综合治疗。结果介入术后12个月,B组输卵管完全通畅率高于A、C、D组,通而不畅率及再闭塞率低于A、C、D组(P<0.05)。结论丹参注射液介入再通联合中医综合治疗对输卵管阻塞性不孕具有良好的临床疗效,丹参注射液术中灌注药物浓度应适当,过高或过低的药物浓度均影响输卵管通畅率。Objective To observe the curative effect and dose-effect relationship of Danshen injection in treating fallopian tube obstruction infertility （FTOI） by fallopian tube recanalization （FTR）and intervention-infusion. Methods 200 FTOI patients were enrolled in this study after successful fallopian tube recanalization by ductus. Patients treated with Danshen injection and combined TCM therapy were dividedinto 3 groups according to different dosage of Danshen injection （group A ： 6 mL; group B ： 10 mL; group C ： 14 mL） . Patients in the control group（group D）were treated with gentamycin sulfate injection, chymotrypsin injection and dexamethasone sodium phosphate injection. A 12-month follow-up survey was performed after FTR to evaluate the rate of tubal patency. Results Inspection at the 12th month after the therapy showed that group B had a higher rate of complete tubal patency than group A, C and D, and the re-occlusion rate and the half-occlusion rate in group B were lower than those in group A, C and D （P 〈 0.05）. Conclusion The therapy involving FTR, injection infusion and combined therapy of TCM for infertility caused by fallopian tube obstruction has favorably clinical efficacy. Concentration of Danshen injection should be appropriate, high or low concentration will affect tubal patency after FTR and curative effect.
