益肾活血通络中药联合来氟米特和甲氨喋呤治疗类风湿关节炎30例疗效观察

摘　　要：目的:比较益肾活血通络中药联合来氟米特和甲氨喋呤与来氟米特和甲氨喋呤联合小剂量激素治疗类风湿关节炎的疗效和不良反应。方法:将60例活动性类风湿关节炎患者按随机数字表法分为两组:实验组30例,口服益肾活血通络中药3次/日,来氟米特(LEF)20mg,1次/日,甲氨喋呤(MTX)10mg,1次/周,泼尼松5mg,1次/日;对照组:口服LEF20mg,1次/日,甲氨喋呤(MTX)10mg,1次/周,泼尼松10mg,1次/日。观察:晨僵时间,握力,关节肿胀数(SJC)关节压痛数(TJC)及休息痛。C-反应蛋白(CRP)、血沉(ESR)、类风湿因子(RF)、肝、肾功、血常规及尿常规,血糖。不良反应:如出现不良反应,应随时详细记录。结论:益肾活血通络中药联合来氟米特及甲氨喋呤和泼尼松5mg,1次/日与来氟米特联合甲氨喋呤联合泼尼松10mg 1次/日疗效相仿,不良反应发生率相当。用益肾活血通络中药治疗组泼尼松用量小而可以达到同样效果,避免长期用较大剂量泼尼松的副反应。Objective：To compare with the therapeutic effect and adverse reaction of rheumatoid rthritis treated with traditional Chinese medicine of tonifying kidney and promoting blood circulation to remove meridian obstruction combined with leflunomide and methopterin between leflunomide and methopterin combined with small doses hormone therapy.Methods：60 cases of active rheumatoid arthritis patients were randomly divided in two groups according to random digits table：30 cases of experimental group received traditional Chinese medicine of tonifying kidney and promoting blood circulation to remove meridian obstruction combined three times per day,leflunomide（LEF）20mg·d^-1,methopterin（MTX）10mg week,prednisone 5mg·d^-1;control group：received LEF 20mg·d^-1,methopterin（MTX）10 mg week,prednisone 10mg·d^-1,observation：time of morning stiffness,grasp force,swollen joint count（SJC）,tender joint count（TJC）and rest pain.CRP,ESR.RF.Liver function.Renal function.Blood routine,urine routine.Blood sugar.Adverse reaction：carefully recorded at any time when adverse reaction presence.Conclusions：the effectiveness of traditional Chinese medicine tonifying kidney and promoting blood circulation to remove meridian obstruction combined with leflunomide,methopterin 5mg·d^-1were similar to leflunomide combined with methopterin,predinisone 10mg·d^-1,their adverse reaction incidence were similar.Treatment group with traditional Chinese medicine of tonifying kidney and promoting blood circulation to remove meridian obstruction used less amount of prednisone but achieved the same effectiveness,avoiding the side effect of long time use of comparatively large amount of prednisone.
