摘 要:目的:探讨血清脂联素(APN)与高血压左心室肥厚(LVH)的关系。方法:高血压患者119例,经超声心动图检查,依据左心室质量指数(LVMI)划分为LVH组(60例)、非LVH组(59例)。采用酶联免疫分析方法测定血清APN浓度。依据血清APN水平将患者分为低水平组(<5.5μg/mL)39例、中间水平组(5.5~7.4μg/mL)40例及高水平组(>7.4μg/mL)40例,应用Logistic回归分析探究血清APN水平与LVH的关系。结果:LVH组血清Log-APN水平显著低于非LVH组〔(0.7±0.1)vs(0.9±0.1)〕μg/mL,(P<0.001)。Logistic回归分析显示与高水平组比较,低水平组发生LVH的OR值为8.56(95%CI:2.51~29.18,P=0.001),中间水平组发生LVH的OR值差异无统计学意义。结论:低APN血症与高血压患者发生LVH相关。Objective:To explore the relationship between serum adiponectin (APN) level and left ventricular hypertrophy (LVH) in hypertensive patients. Methods:According to the diagnosis criterion on hypertension ESH/ESC in 2007( systolic blood pressure SBP≥140 mm Hg (1 mm Hg = 0. 133 kPa) and(or)diastolic blood pressure DBP≥90 mm Hg) from April to October in 2008,119 hospitalized hypertensive patients wene selected. Echocardiographic studies were used to measure various echocardiographic parameters. Left ventricular mass index (LVMI) was calculated using Devereux’s formula (Left ventricular hypertrophy was defined as LVMI≥125 g/m2 for men and LVMI≥110 g/m2 for women). The patients were divided into two groups based on the difference of LVMI: 60 patients with left ventricular hypertrophy,including 37 male and 23 female. Mean of age was(53. 0 ± 12. 2)years. and 59 patients with non-hypertrophy,including 26 male and 33 female. Mean of age was(56. 8 ± 11. 6)years. Blood samples were taken in the the next day morning after patient admission. Enzyme-linked Immunosorbent Assay was used to measure the serum APN level in two groups. t-test,χ2-test ,straight line cor-relation,and multiple regression analysis were used to analyze the results. According to the cutoff points of the serum APN level (5. 5 and 7. 4 μg/mL) patients,were divided into three groups,the lowest category( 5. 5 μg/mL) consisting of thirty-nine cases,the middle category (5. 5-7. 4 μg/mL) consisting of forty cases ,and the highest category ( 7.4 μg/mL) consisting of forty cases. The relationship between LVMI and serum APN level plus confounding factors was analyzed by univariable logistic analysis. Results:Log-APN level was significantly lower in LVH group than in non-LVH group 〔(0. 7 ± 0. 1) vs (0. 9 ±0. 1)〕μg/mL,(P 0. 001). Compared with the highest category,the OR in the lowest category was 8. 56 (95 % CI: 2. 51-29. 18,P = 0. 001). But the OR in the middle categ
