21例GCS 3分特重型颅脑伤病人救治经验

摘　　要：目的总结２１例ＧＣＳ３分特重型颅脑伤病人的救治经验。方法对我科１９９５年１月至１９９７年６月收治的ＧＣＳ３分特重型颅脑伤病人进行回顾性总结。结果２１例ＧＣＳ３分特重型颅脑伤存活９例（４３％），其中恢复良好和中残５例（２３％），重残４例（１９％），死亡１２例（５７％）。结论尽早清除颅内血肿、大骨瓣减压，早期亚低温治疗、呼吸机辅助呼吸、维持内环境稳定及防治并发症等措施。Objective To summarize the therapeutic experience of 21 patients of traumatic head injuries with a score of 3 on GCS. Methods 21 most severe head injuried patients with a score of 3 on GCS who admitted to our department from Jan. 1995 to July 1997 were analyzed retrospectively. Results 9 cases(42%) survived in which 5 cases(23%) had a good recovery or moderate disability, and other 4 cases with severe deficits(19%). The other 12 dead(57%) after therapy. Conclusion The prognosis of most severe head injured patients with a score of 3 on GCS could be improved by early evacuation of intracranial hematoma with large decompressive cranietomies, early moderate hypothermia therapy, early assistent ventilation and effective prevention and treatment of complications.
