某综合性医院冰冻血浆使用情况分析

摘　　要：目的对某综合性医院临床冰冻血浆(Frozenplasma,FP)的使用情况进行调查,分析其合理性,为制订干预措施提供依据。方法按照《临床输血技术规范》中提出的FP输注规范,查阅近半年内的全部FP输注病历,逐份分析其FP使用的合理性,并进行归类统计分析。结果调查期间共有257人次输注FP88000ml,同期血浆与红细胞的输注比例为1∶2.58;全院FP输注总合理率为69.6%,其中手术科室为50.3%,非手术科室为95.5%。FP不合理输注的原因依次为:治疗低蛋白血症、用于扩容、无指征输注或目的不明确、与红细胞搭配输注等。FP使用量较大的科室依次为重症监护病房、感染科、烧伤科,分别占全院FP使用量的46.2%、19.4%、12.7%,合理使用率分别为87.3%、100.0%、9.3%。全院血浆输注剂量不足率为64.2%,其中手术科室为32.4%,非手术科室为86.7%。结论临床FP的使用仍存在不同程度的诸多问题,应采取综合干预措施进一步提高FP使用的合理性。Objective To analyze the clinical usage of frozen plasma（FP）in a general hospital which will help to provide evidences to explore intervention measures.Methods Based on the Technical Criterion of Clinical Blood Transfusion,the cases of FP transfusion of our hospital from Aug.2009 to Jun.2010 were analyzed.Results 88 000 ml of FP were transfused for 257 times and the ratio of FP and RBC is 1：2.58 in the same time.The proportion of reasonable transfusion of FP is 69.6%,and these proportion were 95.5%,50.3% in internalmedicine department and in surgery department,respectively.The causes of the unreasonable transfusion were,in proper order,hypoproteinemia treatment,volume expansion,transfusion without target or following indications,transfusion with RBC together,et al;The top three department of using FP were,in proper order,the intensive care unit,the department of infectious diseases,the burn department,and their ratio were 46.2%,19.4%,12.7% among the all FP,respectively,while the proportion of reasonable transfusion of FP is 87.3%,100.0%,9.3%.The inadequate FP dose rate was 64.2%,and these rate were 86.7%,32.4% in internalmedicine department and in surgery department,respectively.Conclusion There have different degrees problem of transfusion on FP in the hospital and should take some corresponding precautions and measures.
