南海守礁官兵浅部真菌感染病原学分析及治疗

摘　　要：目的调查南海守礁官兵浅部真菌引起皮肤病的真菌分布,分析其发病与工作环境、季节性变化及后勤保障条件的关系。为守礁官兵浅部真菌性皮肤病的防治提供科学依据。方法采用普查法对守岛官兵进行浅部真菌皮肤病流行病学调查,采用湿片法及干片革兰氏染色法镜检,用显色培养基分离,并由ATB全自动微生物分析系统进行鉴定。结果南海驻岛礁官兵1195人普查确诊为皮肤癣病337例,发病率占28.20%。癣病主要分为:脚癣、体股癣、手癣、头癣、甲真菌病;合并两个部位感染68例,占20.17%;浅部真菌感染主要为红色毛癣菌、须癣毛菌、热带念珠菌。结论南海驻岛官兵浅部真菌感染率较以前有所下降,热带念珠菌感染率增高,一年四季感染率无差异,采用药物联合治疗效果好。Objective To investigate the distribution of dermopathic fungus in patients from the arms who stay at reef island of south china sea,evaluate the relations between the invasions of fungus and working environment,season and logistical support status,and provide the scientific basis for the treatment and prevention superficial mycosis.Methods By using census method the prevalence of superficial mycosis in the arms who stay at reef island of south china sea was investigated.The wet sheet glass method and Gram staining method were used to inspect the germ,the coloration differential medium was used to separate the germ,and the ATB system was sued to identification of germs.Results The dermatophytid,including tinea pedis,tinea corporis and cruris,tinea manum,tinea capitis and tinea unguium,was founded in 337 of 1 195 soldiers from the troops stationed on reef islands in south China sea（accounted for 28.20%）.There were 68 people with two or more kinds dermatophytid（20.17%）.The main disease fungi causing superficial mycosis in soldiers were trichophyton rubrum,trichophyton mentagrophytes and candida tropicalis.Conclusion The infection rate of superficial mycosis in the troops stationed on reef islands in South China Sea is lower than before.The infecting rate of candida tropicalis is high and shows no seasonal changes.The combination therapy can achieve a best curative effect.
