应用睾丸鞘膜一期修复尿道下裂的新认识

摘　　要：目的探讨应用睾丸鞘膜重建尿道的可行性。方法本组17例尿道下裂患者,其中15例同时合并有睾丸鞘膜积液,其余2例同时合并有隐睾,前者采用带蒂睾丸鞘膜一期修复尿道,后者采用游离睾丸鞘膜一期修复尿道。观察其术后效果并进行临床分析。结果除1例应用带蒂睾丸鞘膜者发生尿瘘外,余16例均获痊愈,无尿道狭窄,阴囊阴茎外观满意,阴茎伸展自如。结论带蒂睾丸鞘膜血供好,重建尿道修复同时合并有睾丸鞘膜积液的尿道下裂疗效确切,操作简单。用游离睾丸鞘膜重建尿道修复同时合并有隐睾的尿道下裂也能取得较好的效果。Objective To explore the feasibility of reconstruction urethra with perididymis skin flap.Methods To study 17 patients with hypospadias,among which 15 patients meanwhile complic hydrocele testis,the other 2 meanwhile complic cryptorchidism.The former‘urethra first phase were repaired by perididymis pedicle skin flap,and the latter‘urethra first phase were repaired by Lib perididymis.Results One patient concidenced urinary fistula,the other 16 recovered without urethral stricture and their angles of penis to scrotum were normal.Conclusions The flap is easily developed with reliable blood supply.Reconstruction urethra is a simple,convenient and effective technique for hypospadias complic hydrocele testis.To hypospadias complic cryptorchidism,also it would effect well to reconstruction urethra by Lib perididymis.
