保守及手术治疗稳定型胸腰椎压缩性骨折的对照研究

摘　　要：目的探讨过伸复位朱厄特型胸腰骶椎矫形器外固定法、经皮椎体成形术法及传统垫枕疗法三者在治疗稳定型胸腰椎压缩性骨折临床疗效的差异。方法将120例患者随机分为三组,即A组40例采用垫枕复位法治疗;B组40例采用过伸复位朱厄特型胸腰骶椎矫形器外固定法治疗;C组40例采用经皮椎体成形术法治疗。所有患者均于复位前、后、复位后1、3及6个月记录压缩椎体的压缩率、复位率和复位6个月后临床症状体征评分。结果与A组比较,B、C组在复位后、复位后1、3及6个月受伤椎体压缩率、复位率和复位6个月后临床症状体征评分均具有统计学意义(P<0.05),而B、C两组之间比较无统计学意义(P>0.05)。结论与传统垫枕复位相比,过伸复位朱厄特型胸腰骶椎矫形器外固定法和经皮椎体成形术法治疗稳定型胸腰椎压缩性骨折均具有较好的临床疗效,但经皮椎体成形术法与过伸复位朱厄特型胸腰骶椎矫形器外固定法在治疗稳定型胸腰椎骨折的临床疗效上无统计学意义。Objective To investigate the clinical efficacy of over the reset Jewett hyperextension-type thoracolumbar orthosis external fixation, percutaneous vertebroplasty and traditional pillow therapy cure the stable thoracolumbar compression fracture. Methods one hundred twenty patients were randomly divided into three groups, namely A group of 40 cases treated with pillow repositioning; B group of 40 cases with reset Jewett hyperextension-type thoracolumbar orthosis external fixation; C group with 40 cases by Percutaneous vertebroplasty treatment. All patients were record of vertebral compression rate, reset rate in the reset before and after the reset 1,3 and 6 months after the injury and reset after 6 months of clinical symptoms and signs in the ratings. Results B, C group injured reset after reset and after 1,3 and 6 months, vertebral compression rate, reduction rate and the rate of clinical symptoms and signs had a significant difference （P〈0.05）, while the B, C comparison between groups had no significant differences （P^O.05）. Conclusion Compared with the traditional pillow reset, reset Jewett hyperextension-type thoracolumbar orthosis external fixation and percutaneous vertebroplasty in treating stable thoracolumbar vertebral compression fractures have a better clinical efficacy, while there is no difference between brace treatment and percutaneous vertebroplasty.
