清热利胆合剂治疗肝胆湿热型慢性胆囊炎70例疗效观察

摘　　要：目的观察清热利胆合剂治疗肝胆湿热型慢性胆囊炎的临床疗效。方法选取肝胆湿热型慢性胆囊炎150例,随机分为2组,治疗组70例予清热利胆合剂50mL,每日2次口服;对照组80例予胆石通胶囊6粒,每日3次口服。2组均2个月为1个疗程,连续治疗2个疗程观察临床疗效。并另设健康对照组30例,比较3组治疗前后血浆肿瘤坏死因子(TNF-α)、β-内啡呔(β-EP)及血清丙二醛(MDA)、血清超氧化歧化酶(SOD)变化。结果治疗组总显效率为65.7%,总有效率98.6%;对照组总显效率62.5%,总有效率96.3%,2组比较差异均无统计学意义(P>0.05)。治疗前治疗组和对照组TNF-α、β-EP及MDA均增高,SOD下降,与健康对照组比较差异均有统计学意义(P<0.05);治疗后治疗组和对照组TNF-α、β-EP及MDA均下降,血清SOD上升,与本组治前比较差异有统计学意义(P<0.05),且治疗组优于对照组(P<0.05)。结论清热利胆合剂具有抗炎、镇痛、抗氧自由基损伤的作用,治疗肝胆湿热型慢性胆囊炎疗效确切。Objective To observe the effect of Qingre Lidan mixture on chronic cholecystitis with dampness-heat of liver and gallbladder type. Methods 150 patients with chronic cholecystitis with dampness-heat of liver and gallbladder type were randomly divided into two groups. 30 healthy persons were as blank group. Patients in treatment group （ n = 70） were treated by Qingre Lidan mixture,twice a day. 80 cases in control group were treated by Danshitong capsule,6 pills,thrice a day. The therapeutic course was two months in two groups. TNF α,β EP,MDA,SOD and effect were observed after two courses. Results The total excellence rate and total effective rate in treatment group （65. 7% ,98. 6% ） were same as those in control group （62. 5% ,96. 3% ,P〈 0. 05）. Compared with blank group,TNF α,β EP and MDA before treatment was increased in two groups,and SOD was decreased （ P〈 0. 05）. TNF α,β EP and MDA after treatment was decreased in two groups,and SOD was increased （ P〈 0. 05） compared with blank group. Conclusion Qingre Lidan mixture has the effect of anti-inflammatory,relieving pain and antioxygen free radical damage,and definite effect on chronic cholecystitis with Dampness heat of liver and gallbladder type.
