早产儿脑室周围-脑室内出血相关因素研究

摘　　要：目的采用床边头颅B超早期诊断早产儿脑室周围-脑室内出血(PVH-IVH),分析其相关危险因素。方法住院的443例早产儿为研究对象,Logistic回归分析影响PVH-IVH的危险因素。结果单因素分析显示:胎龄、出生体质量、机械通气、血细胞比容、血红蛋白、低体温、生后感染、呼吸暂停、PaCO2在PVH-IVH组和无PVH-IVH组之间差异有统计学意义(P<0.05)。Logistic回归分析显示:胎龄低、机械通气和血红蛋白水平低是早产儿PVH-IVH的危险因素,胎龄低和呼吸暂停是重度PVH-IVH的危险因素。结论胎龄低、机械通气和血红蛋白水平低是早产儿发生PVH-IVH的危险因素。Objective Using the skill of head ultrasound to make a early diagnosis for preterm infants with periventricular-intraventricular hemorrhages（PVH-IVH）,to analyze some related risk factors that could result in PVH-IVH.Methods 443 preterm infants were chosen,who were born during Jan 2006 and Jun 2009.The factors of PVH-IVH were analyzed by Logistic regression.Results Univariate factor analysis showed that the difference were significant（P〈0.05 or P〈0.01） in gestational age,birth weight,mechanical ventilation,hematocrit,hemoglobin,hypothermia,apnea,postnatal infection and PaCO2 between PVH-IVH group and non PVH-IVH group.Multiple factors Logistic regression showed that low gestational age,mechanical ventilation and low hemoglobin were the risk factors of PVH-IVH.Besides,low gestational age and apnea are risk factors of severe PVH-IVH.Conclusion The risk factors for PVH-IVH of preterm infants are low gestational age,mechanical ventilation,and low levels of hemoglobin.Preterm infants with low gestational and history of apnea have higher incidence of severe PVH-IVH.
