摘 要:目的探讨全前脑畸形的产前超声特征及其诊断价值。方法运用胎儿系统检查法对胎儿进行畸形筛查,对检出全前脑畸形病例的超声表现与引产后尸检结果进行比较、分析。结果共对1 0258例孕妇进行畸形筛查,检出5例全前脑畸形,5例颅内结构产前超声特征为脑中线结构消失,无叶全前脑为单一脑室,半叶全前脑后角分开,两侧丘脑融合或部分融合。所有5例均伴有不同程度颜面部畸形,共10个,2例合并其他畸形共4个。与引产后尸检结果比较,所有颜面部畸形均检出,1例合并多趾畸形漏诊。结论产前超声是诊断无叶及半叶全前脑畸形的可靠有效方法。Objective To analysethe ultrasonographic feature of Holoprosencephaly and investigate its diagnostic value.Methods Using the check methods of fetal system to screen the fetus malformation,and compare the ultrasonographic feature of Holoprosencephaly to autopsy results of post-abortion correspondingly.Results 10,258 cases of pregnant women were screened,and 5 were detected holoprosencephaly which characterized by the disappearance of midline structures,alobar holoprosencephaly as a single lateral ventricle,semilobar holoprosencephaly with posterior horn separated and two sides of thalamus anastomosed wholly or partly.There were 10 facial malformations in all 5 cases and 4 other malformations in 2 cases.Compared with the autopsy results,all the facial deformities were detected unanimously except one combined with multiple toe missed.Conclusion The prenatal ultrasound can accurately show the intracranial structural abnormalities as well as facial abnormalities,which has definitive diagnosis and differential diagnosis in alobar/ semilobar holoprosencephaly.
