摘 要:目的探讨系统性红斑狼疮(SLE)患者妊娠对胎、婴儿影响和幼儿智力发育状况影响。方法对80例SLE妊娠的临床资料作前瞻性分析,分析新生儿存活情况,45例幼儿采取DDST筛选智能。结果80例SLE孕妇获婴儿81例(双胎1例),均存活。18个月至7岁半幼儿共45例进行DDST筛选,其中智力异常2例(0.4%)。结论妊娠期间强的松适量口服及妊娠期SLE恶化时增加剂量和干预性早产是降低围产儿死亡的关键。SLE妊娠与正常妊娠的幼儿智力异常分别为0.4%及0.8%,二者无统计学差异。Objective To estimate the effect of SLE complicating pregnancy on fetus and infanst and on their intelligence development. Methods To carry out a prospective analysis on viability of neonates in 80 pregnant women compelicated by SLE and compare their intelligence development with those of 45 newborn infants using Denven development screening test (DDST). Results 81 viable neonates were delivered by 80 gravid suffering from SLE.(one of these patients gave birth to a twin) 45 infants' intelligence from 18 month old to 7.5 year old were tested by DDST. Two of them were abnormal (The abnormal rate was 0.4%). Conclusions Using suitable dosage of prednisone when SLE complicates poregnancy and increasing the dosage of prednisone when the disease becomes worse is the key measure of reducing the perinatal morbidity. The rate of infants' abnormal intelligence development in SLE complicating pregnancy is 0.4%, where as in normal pregnancy it is 0.8%, There is no statistical difference between the two.(Shanghai Med J, 1999,22:157 159)
