布托啡诺联合舒芬太尼用于腹部肿瘤患者术后自控静脉镇痛效果的观察

摘　　要：目的:研究布托啡诺联合舒芬太尼用于腹部肿瘤患者术后自控静脉镇痛(Patient-Controlled IntravenousAnalgesia,PCIA)的临床效果与安全性。方法:将我院90例腹部肿瘤术后患者随机分为三组(每组各30例)行自控静脉镇痛。布托啡诺组(B组):布托啡诺12mg;舒芬太尼组(S组):舒芬太尼150μg;布托啡诺联合舒芬太尼组(BS组):布托啡诺4mg+舒芬太尼100μg。三组均加入胃复安30 mg及生理盐水总共配制成200ml。监测并记录用药后4、8、12、24、48小时患者的镇痛效果(采用VAS评分)和镇静效果(采用Ramsay评分)、不良反应及总的患者自控镇痛(Patient-Controlled Analgesia,PCA)按压次数。结果:三组患者镇痛效果均确切可靠,三组患者总的PCA按压次数差异无统计学意义。12小时后B组VAS评分显著高于S组和BS组(P<0.05),12小时后B组Ramsay评分显著高于S组和BS组(P<0.01),用药全程S组和BS组VAS评分及Ramsay评分均无统计学意义(P>0.05)。不良反应显示B组头晕较S组和BS组明显增多(P<0.05)。结论:布托啡诺联合舒芬太尼具有与单用舒芬太尼相似的镇痛作用,而不良反应相对较少,可以作为临床术后镇痛的一种有效选择。Objective： To evaluate the PCA effects and safety with Butorphanol combining Sufentanil for abdominal tumor patients. Methods ： 90 cases of post-operative abdominal tumor patients in our hospital treated with PCA were randomly allocated into 3 groups （30 cases in each group） ： Butorphanol group （ Group B） , using Butorphanol 12mg; Sufentanil group （ Group S）, using Sufentanil 150μg; and Butorphanol combining Sufentanil group （Group BS） ： using Butorphanol 4mg ＋ Sufentanil 100μg; with which Metoclopramide 30 mg was mingled in saline for 200ml regime. The VAS scores, Ramsay scores, adverse effects and PCA numbers were observed and recorded at time points of 4, 8, 12, 24, and 48 hour after operation. Results： The analgesia effects were concrete and reliable in all three groups. There was no significant difference in PCA numbers. The VAS score and Ramsay score in Group B was significantly higher than that in Group S and Group BS at 12 hour after operation（ P 〈 0. 05 and P 〈 0. 01, respectively）. At all time points during PCA, there was no significant difference for the VAS scores and Ramsay scores in Group B and Group BS （ P 〉 0.05 ） , and the occurring rates of dazzling in Group B were significantly higher than that in Group S and Group BS （ P 〈 0. 05 ）. Conclusion ： The PCA regime of Butorphanol combining Sufentanil has the similar analgesia effects as that of Sufentanil alone, with relatively lower adverse effect rate, which could be used as an effective alternation.
