摘　　要：目的探讨主动脉-左室隧道(ALVT)的超声声像图特征及超声诊断价值。方法对5例经心血管造影对照,4例经手术证实的超声心动图诊断为ALVT患者的超声图像特征进行回顾性分析,并结合文献探讨ALVT的超声声像图特征和超声心动图诊断ALVT的价值。结果 ALVT超声心动图表现典型:二维超声心动图示主动脉瓣周见一管状无回声腔,在主动脉瓣环下方进入左室流出道,彩色多普勒血流显像示收缩期血流信号从左室流出道经管状无回声腔进入主动脉,舒张期于瓣下探及一束五彩血流从主动脉异常管状无回声腔至左心室。5例ALVT中超声诊断准确4例,误诊为右冠状动脉左室瘘1例。结论 ALVT的超声声像图特征明显,超声心动图对ALVT有重要的诊断价值。Objective To explore the ultrasonographic characteristics of aortic left ventricular tunnel （ALVT） and evaluate the value of echocardiography in the diagnosis of ALVT.Five cases diagnosed as ALVT by ultrasonography were reviewed,and compared with the angiocardiography results.4 cases of them were confirmed by operation.The features of ultrasonographic image were obvious as fellows：the two-dimensional echocardiography image showed that the non-echo tubal cavity around the aortic vavle connected with left ventricular outflow-tract under the aortic valve annulus.Color Doppler Flow Imaging showed that the blood flow sign from left ventricular outflow-tract entered into the aorta through the non-echo tubal cavity during systolic phase,and vice versa during diastolic phase.4 cases were accurately diagnosed as aortic-left ventricular tunnel by ultrasonography firstly.1 cases were misdiagnosed as right coronary artery left ventricular fistula.The feature of ultrasonographic image are obvious and ultrasonography plays an important role in diagnosis of ALVT.
