奈替米星在烧伤患者体内药代动力学研究

摘　　要：目的：测定奈替米星（ｎｅｔｉｌｍｉｃｉｎ，ＮＥＴ）在１０名烧伤患者体内药代动力学参数、渗出液药物浓度和尿药浓度。方法：采用荧光偏振免疫法测定一次静滴６ｍｇ·ｋｇ－１奈替米星注射剂后不同时间的血、尿、烧伤渗出液中药物浓度，用３Ｐ９７计算药代动力学参数。结果：烧伤患者静滴６ｍｇ·ｋｇ－１ＮＥＴ后ｔ１／２为（２４０±１０９．９８）ｍｉｎ；Ｃｌｓ为（０．０９７３±０．０１９８）ｍｇ·ｍｉｎ－１·μｇ－１·ｍｌ－１；用药８ｈ后血浓度为（１．５６±０．６６）μｇ·ｍｌ－１；１２ｈ尿排出率（７３．３６±１０．０４）％；２４ｈ尿排出率（８３．３９±９．４６）％。结论：奈替米星（６ｍｇ·ｋｇ－１）用于烧伤患者可有效控制烧伤后感染，但体内药代动力学差异较大。OBJECTIVE:To determine the pharmacokinetic parameters and concentrations of netilmicin in the blood,exudate and urine in 10patients with burns.METHOD:The pharmacokinetic parameters and concentrations of Netilmicin in the blood,urine and exudate were determined by FPIA.Netilmicin was given at a single dose of 6mg·kg -1 by intravenous infusion.RESULTS:The main pharmacokinetic parameters were t 1/2 =(240.28±109.98) mins, Cls =(0.0973±0.0198)mg·min -1 ·μg -1 ·ml -1 .The mean blood concentration was (1.56±0.66)μg·ml -1 at 8h after administration.The elimination rates of netilmicin in urine were (73.36± 10.04 )% and (83.39±9.46)% after 12 h and 24 h respectively. CONCLUSION:Netilmicin(6mg·kg -1 )can contre the burn infetion,but the pharmacokinetic parameters are varied in different individuals,the blood concentration must be monitored.
