Wallis腰椎弹性固定术治疗腰椎退变性腰痛疗效观察

摘　　要：目的观察Wallis棘突间动态稳定系统治疗腰椎退变性腰痛的疗效。方法 2007年10月～2008年12月共使用Wallis棘突间动态稳定系统治疗腰椎退变性腰痛患者37例,男27例,女10例,年龄19～75岁,平均48.0岁。手术开窗摘除突出的间盘,置入Wallis系统。对患者术前、术后1月、术后1年进行VAS及ODI评分。术后随访(12.1～25.4)月,平均(17.9±4.0)个月。结果 1例术后半年出现腰痛复发,保守治疗缓解。VAS及ODI术前评分(7.8±1.0)分,(77.9±4.8)%;术后1月评分(1.6±1.0)分,(24.2±4.6)%;术后1年评分(0.8±0.7)分,(13.0±3.8)%。运用SPSS13.0分析,P<0.01,改善均具有统计学意义。结论 Wallis系统作为一种弹性固定系统,能够保留固定节段一定的活动度,维持腰椎稳定,预防临近节段退变,是治疗腰椎退变性腰痛的有效手段。Objective To observe the therapeutic effect of Wallis interspinous dynamic stabilization system in the treatment of lumbar degenerative disease. Methods During October 2007 to December 2008, 37 patients （39 intervertebral spaces） with degenerative low back pain were treated with Wallis system. There were 27 males and 10 females with the average age of 48.0 years （ranging from 19 to 75 years）. The Wallis system was implanted after the excision of herniated disc by laminotomy. The VAS and ODI scales were given before, one month and one year after the surgery. The follow-up duration was （17.9±4.0） months on average, ranging from 12.1 to 25.4 months.Results One patient had recurrent low back pain which was alleviated by conservative treatment. Before, one month and one year after surgery, VAS and ODI scores were （7.8±1.0） and （77.9±4.8）%, （1.6±1.0） and （24.2±4.6）%, and （0.8±0.7） and （13.0±3.8）%, respectively. Statistical analysis by SPSS 13.0 revealed the significant improvements （P〈0.01）. Conclusion The Wallis interspinousn system, as a dynamic stabilization fixation, which can preserve the mobility of the fixed segment, sustain the lumbar stabilization, prevent adjacent segment degeneration, is an effective instrument to treat degenerative low back pain.
