摘 要:目的探讨高血压患者血清中抗α1肾上腺素受体自身抗体(α1-AA)的分布特征及其病理意义。方法采用酶联免疫吸附测定(ELISA)技术,检测86例高血压患者与100名健康对照者血清中α1-AA的滴度和阳性率;利用亲和层析法纯化α1-AA阳性和阴性患者血清中的IgGs,观察该抗体IgGs对大鼠离体胸主动脉血管环张力的影响。结果高血压患者血清中α1-AA的阳性率和平均抗体滴度分别为49%和1∶(93.5±2.4),明显高于健康对照组的9%和1∶(16.6±3.8),差异有统计学意义(P<0.01)。纯化的抗体阳性IgGs(10-9~10-5mol/L)能够浓度依赖性增加大鼠离体胸主动脉血管环的张力[收缩幅度从(19.3±2.2)%增加到(137.5±4.6)%,P<0.01],这种作用与α1-肾上腺素受体(AR)激动剂去氧肾上腺素的缩血管效应相类似,并且呈现非血管内皮依赖性,同时可被α1-AR拮抗剂哌唑嗪所阻断,以及被α1抗原肽段中和。结论高血压患者血清中α1-AA的分布呈高阳性率与高滴度的特征,该抗体具有激动剂样增加离体胸主动脉血管环张力的作用,并且为非内皮依赖性,可能是通过与血管平滑肌细胞的α1-AR结合所实现。Objective To investigate the distribution and pathological significance of autoantibody against α1-adrenoceptor (α1-AA) in patients with hypertension. Methods SA-ELISA assay was used to detect the α1-AA from sera of 86 hypertension patients and 100 normal controls; IgGs from the positive sera and negative sera of hypertension patients were prepared using a MabTrap Kit (Amersham) following the manufacturer' s instructions. The biological effects of α1-AA on the tension of isolated thoracic aortic rings from rats were also observed. Results The positive rate and titer of α1-AA in the sera of patients with hypertension were obviously higher than those from normal control [49%, 1∶(93.5±2.4)] vs [ 9%, 1∶(16.6±3.8)](P0.01). The IgGs from α1-AA positive sera (10-9~10-5 mol/L) increased ten sion of isolated thoracic aortic rings from rats in a concentration-dependent manner [maximal contraction from (19.3 ± 2.2) % to (137.5±4.6) %, P0.01]. This action was similar to performance of α1-AR agonist phenylephrine (PE), independent of endothelial cells, and can be blocked by Prazosin, an α1-AR agonist, and can also be neutralized by α1 antigenic peptide fragments. Conclusion The characteristics of α1-AA distribution in sera of hypertension patients showed a high-positive rate and high titer. The α1-AA exhibited an ′agonistic-like′ vasocontraction effect which was independent of endothelial cells and may be attributed to its binding to α1-AR in vascular smooth muscle.
