CO_2激光联合盐酸氨酮戊酸散光动疗法治疗尖锐湿疣的疗效观察

摘　　要：目的观察CO2激光联合外用盐酸氨酮戊酸散光动疗法(aminolevulinic acid hydrochloride for topical powderphotodynamictherapy,ALA-PDT)治疗尖锐湿疣的临床疗效与安全性。方法治疗组36例患者,经CO2激光术后联合ALA-PDT,1周治疗1次,连续治疗3周;对照组28例患者,经CO2激光术后,联合使用咪喹莫特乳膏涂抹患处,1周3次,连续使用8周。2组患者均在末次治疗后第1周进行疗效评价和不良反应观察,第4～8周观察复发率。结果末次治疗1周时,2组疣清除率均为100%,无统计学差异(P>0.05);2组均无全身不良反应,局部不良反应治疗组为13.89%,主要为红肿、疼痛、渗液,对照组为64.29%,主要为疼痛、糜烂、红肿、红斑等,具统计学差异(P<0.05)。结论 CO2激光联合ALA-PDT治疗尖锐湿疣,清除速度快,疣体清除率高,复发率低,安全,耐受性好,无明显副作用,值得临床推广。Objective To observe the clinical efficacy and safety of the CO2laser therapy combined aminollevlinic acid Hydrochloride for topical powder photodynamictherapy to treat condyloma accuminatum.Methods A total of 36 patients were studied in the treatment group,combined ALA-PDT after the CO2laser therapy,once a week for 3weeks.And 28 patients in the contrast group,combined imiquimod cream after the CO2laser therapy,three times a week for 8 weeks.We observe the efficacy and adverse reaction in the first week after the last treatment,and also the recurrence rate at 4to 8 weeks.Results The first week after the last treatment,the clearance was 100%,with no statistically significant difference in the 2groups（P0.05）.The recurrence in the treatment group was 5.56%,and that of the control group was 7.14%,with no statistically significant difference（P0.05）.There were no systemic adverse re-actions in the 2groups.Local reactions was 13.89%for the treatment group,including swollen,pain,and drainage,and 64.29%for the contol group,including pain,anabrosis,swollen,and erythema,it has significantly difference（P0.05）.Conclusion CO2laser therapy combined ALA-PDT for condyloma accuminatum is high clearance speed,and low recurrence rate.It is safe,tolerable,with no obvious side effects.
