2型糖尿病肾病患者红细胞免疫功能研究

摘　　要：目的探讨2型糖尿病肾病患者红细胞免疫功能的变化及与空腹血糖、糖化血红蛋白和胰岛素的相关性。方法对26例2型糖尿病肾病变患者红细胞C3b受体花环率、红细胞免疫复合物花环率、红细胞免疫黏附促进率及红细胞免疫黏附抑制率进行了检测,同时测定了患者的空腹血糖、糖化血红蛋白和胰岛素水平,并与21例健康者进行了对比分析。结果 2型糖尿病肾病患者RBC-C3bRR及免疫黏附促进率降低,与FPG、HbA1c及In呈负相关;RBC-ICR增高,与FPG、HbA1c及呈正相关;免疫黏附抑制率及增高与FPG及HbA1c呈正相关,而与胰岛素无相关性。结论 2型糖尿病肾病患者红细胞免疫功能处于低下是导致2型糖尿病肾病重要因素,与血糖控制不佳有关。Objective To study the changes of erythrocytes immune function in patients with nephropathy of Type 2 Diabetes Mellitus（T2DM） ,the correlation to the levels of fasting plasma glucose （FPG） and glycated hemoglobin A1c （HbA1c） and insulin.Methods RBC-C3b receptor rosette rate （RBC- C3bRR） and RBC-immune complex rosette rate （RBC-ICR） and rosette forming enhancement rate （RFER） and rosette forming inhibitor rate （RFIR） were detected,the concentrate of FPG,HBA1c and insulin were also determined in 26 patients with nephropathy of T2DM（DN）as well as 21 controls.Results The levels of RBC - C3bRR and RFER in patients were significantly lower than those in controlsend exhibited a negative linear correlation to FPG and HbA1c and insulin levels.RFIR level in patients was remarkably higher than that in controls and exhibited a positive linear correlation to FPG and HbA1c levels;The levels of RBC-C3bRR and RFER in patients were significantly lower than those in controlsend exhibited a negative linear correlation to FPG and HbA1c and In levels;RBC-ICRR level in patients was remarkably higher than that in controls and exhibited a positive linear correlation to FPG and HbA1c and In levels;RFIR level in patients was remarkably higher than that in controls and exhibited a positive linear correlation to FPG and HbA1c levels,but no correlation with insulin.Conclusion The lowered red cell immune function is an important factor leading to diabetic nephropathy,and associated with the poor glucose control.
