卡维地洛对高血压左室肥厚作用的超声心动图评价

摘　　要：【目的】应用超声心动图评价卡维地洛对高血压患者左室肥厚的影响。【方法】选择2009年1月-2009年7月门诊轻中度高血压患者35例,符合1-2级原发性高血压诊断,口服安慰剂2周后,给予卡维地洛12.5 mg口服治疗,疗程为24周,剂量每2周递增12.5 mg,最大剂量50 mg,观察诊室坐位血压变化,和24周超声心动图检查对左室肥厚的影响的变化。【结果】应用卡维地洛治疗24周后,患者血压明显降低,E/A升高,同时心室肥厚指标显著降低。卡维地洛治疗24周后左室重量与治疗前患者舒张压水平及治疗前左室重量有关。【结论】卡维地洛降压的同时可以改善左室舒张功能,有逆转左室肥厚的趋势。[Objective] To observe the effect of carvedilol on hypertension and left-ventricular hypertrophy by echocardiography. [Methods] Thirty-five clinical patients with light or moderate hypertension were chosen according to the diagnosis of Ⅰ-Ⅱ primary hypertension as the observation group from January to July,2009. For the frist 2 weeks they were treated with a placebo,then for the next 24 weeks with earvedilol,at a dose of 12.5 mg for consolidation and observation.There was an optional addation 12.5 mg over a 2 weeks period.The maximal dose was somg.Obervation was the change of clinical sitting blood-pressure, and 24 weeks left-ventricular hypertrophy in echocardiography was observed. [ Results] The clinic blood-pressure decrease and E/A increas were noticeable. At the same time, the index of left-ventrieular hypertrophy declined remarkably after earvedilol was used for 24 weeks.The weight change of left ventricle was related to the diastolic pressure and the weight of left ventride before treatment. [Conclusions] Carvedilol can treat the mild to moderate hypertension effectively.It can improve left-ventrieular diastolic function and reverse the tendency to left-ventricular hypertrophy.
