摘  要:目的:探讨磁共振扩散加权成像(DWI)对前列腺疾病的诊断与鉴别诊断价值。方法:选取我院收治的118例前列腺疾病患者,其中,BPH患者71例,前列腺癌患者47例。所有患者均进行磁共振常规扫描和DWI扫描,分析前列腺增生(BPH)和前列腺癌的DWI和表观扩散系数(ADC)图表现,并测量癌肿区的ADC值及BPH的平均ADC值,进行统计学分析。结果:BPH的DWI和ADC图上外周带信号均匀,中央腺体区信号不均匀;前列腺癌病灶在DWI上呈高信号,ADC图呈低信号,受累的精囊,淋巴结转移和骨转移病灶DWI上呈高信号,ADC图上呈低信号;前列腺癌的ADC值(78±16)×10^(-5)mm^2/s,BPH组的平均ADC值为(146±44)×10^(-5)mm^2/s,两者差异有统计学意义(P<0.05)。结论:DWI检查在前列腺相关疾病诊断中有特征性表现,并对前列腺增生和前列腺癌具有鉴别诊断价值。Objective:Diagnosis and differential diagnosis of magnetic resonance diffusion-weighted imaging of prostate disease. Methods:We select admitted 118 patients of our hospital with prostate disease,including 71 cases of patients with BPH,47 cases of prostate cancer.All patients were carried out conventional MRI and DWl MRI scans,analyze prostate hyperplasia (BPH) and DWI of prostate cancer and apparent diffusion coefficient (ADC) map the performance and measure the cancer area,the average ADC values and ADC values of BPH to carry out statistical analysis.Results:DWI of BPH and ADC map peripheral band signal uniformity, the central gland area signal uneven;prostate cancer lesions on DWI on the high signal intensity,ADC map showed low signal involvement of the seminal vesicle,lymph node metastasis and bone metastasis DWI last showed a high the signal,ADC map showed low signal.The ADC values of prostate cancer (78±16)×10^-5mm^2/s,BPH group,the average ADC values (146±44)×10^-5mm^2/s,difference was statistically significant (P〈0.05).Conclusion:DWl examination in the diagnosis of prostate-related diseases have characteristic performance,as well as benign prostatic hyperplasia and prostate cancer have a differential diagnostic value.
