摘　　要：目的:本项研究利用计算机技术设计虚拟神经外科脑室穿刺手术模型。方法:将我国正常成年人脑室形态数据、虚拟可视(韩国)人(VKH)的头部数据与Talairach脑图谱数据进行配准和三维重建,在仿真颅脑模型上应用机械臂操作系统建立与临床手术操作完全相同的脑室穿刺手术模型,计算机显示系统随时显示穿刺针进针线路、方向、速度和位置,记录操作者操作经过并形成报告。结果:选择10名低年资非专科医师经该套手术模拟设备洲练,模拟脑室穿刺手术成功率较训练前明显提高(P<0.01);穿刺角度数据进行T-TEST检验,显示脑室穿刺成功率与进针角度有显著相关性(p<0 05)。结论:本项研究显示虚拟现实技术在脑室穿刺手术上的应用,为临床医师特别是基层医院年轻专科医师,快速、熟练掌握脑室穿刺手术技巧,提供安全、有效的训练手段。Objective：This study is focus on designing neurosurgical simulation system for ventricular puncture using virtual reality technology.Methods：Normal Chinese adult ventricular data,CT data of the Virtual Korea Human （VKH） and Talairach brain atlas are made registration,and three-dimensional human head model are reconstructed.The virtual surgery simulation system for ventricular puncture with haptic feedback is established according to identical clinical ventricular puncture operation.The trajectory,direction,velocity and position during puncturing are also shown and recorded as the users＇ training report.Results：Ten interns were invited to participate in the evaluation of the established virtual ventricular puncture simulation system.Results shown that after training,the success rate of ventricular puncture has significantly increased （P〈0.01） and the success rate has a significant correlation with needle direction （T-Test,P〈0.05）.Conclusion：This research has studied the initial result of using virtual-reality based ventricular puncture simulation system for training interns.It can be used as a safety and effective training method to help younger physicians improve their surgical skills of ventricular puncture.
