妊娠期急性女阴溃疡1例

摘　　要：患者女,24岁,妊娠21+1周。外阴反复疼痛性溃疡2年,加重1月。患者在怀孕前曾有外阴亚急性、慢性、复发性溃疡病史,怀孕期间右侧大阴唇出现坏疽型溃疡,有触痛。溃疡分泌物涂片革兰染色见较多革兰阳性粗大杆菌及少许芽生孢子。皮损组织病理示:表皮增厚,海绵水肿,真皮全层及皮下脂肪小叶弥漫淋巴细胞为主浸润,部分区域嗜中性白细胞浸润,未见血管炎表现。诊断为急性女阴溃疡(坏疽型),经依替米星0.2g1次/d静脉注射及强的松30mg/d口服治疗1周后好转并中止妊娠。The patient was a 24-year old female who＇s pregnant for 24 ＋1 w. She had a history of subacute, chronic, re- current vulvar ulcer hefore pregnancy and suffered a gangrenous ulcer during the pregnancy. The smear of the ulcer seeretiun reveals many G ^＋ bulky bacilli with some blastospores. Histopathology shows thicker epidermis with spongiosis, diffuse infiltration of the entire dermis and subcutaneous fat lobules with neutrophil infiltration in some regions while no manifestations of vasculitis. The diagnosis was Ulcus Vulvae Acutum which had been relieved by a combination therapy of etimicin 0.2g/d and prednisone 30mg/d for 1 week followed by a termination of the pregnancy.
