摘  要:目的:观察越婢汤对阿霉素肾病(adriamycin nephropathy,AN)大鼠肾小球超微结构的影响。方法:尾静脉注射阿霉素6mg/kg建立AN大鼠模型,并随机分为空白组(A组)、模型组(B组)、越婢汤(C组)、激素(D组)、苯那普利(E组),给予相应干预;第3周、7周留取肾皮质,以醋酸铀和柠檬酸铅双重染色法,H-600透射电镜观察GBM超微结构,并计数单位肾小球基底膜上足突个数。结果:(1)造模成功后与A组相比,模型组大鼠24h尿蛋白显著升高(P<0.01),血清Alb明显下降(P<0.01),血清TG和TC明显升高(P<0.01)。(2)模型成功时透射电镜示模型组大鼠肾小球足突呈部分或弥漫性融合,单位长度基底膜上足突计数明显减少。(3)干预结束后与B组相比,C、D组超微结构明显改善,足突计数明显增加,且排列相对整齐(P<0.01)。结论:越婢汤可改善AN大鼠肾小球超微结构,从而改善修复肾小球电荷屏障。Objective:To investigate the variation of glomerular ultrastructure in adriamycin-induced nephropathy (AN) rats,and to further explore the effects of Yuebi Decoction on proteinuria in AN rats.Methods:Adriamycin nephropathy was induced by a single tail intravenous injection of adriamycin.Seventy rats were randomly divided into the normal control group(A,n=16)and the nephropathy model group(AN,n=54).Three weeks later,the nephropathy models were established,and 16/14 rats were sacrificed while the rest of 40 AN rats randomly divided into four groups:the nephropathy model group (B,n=10),the yuebi group(C,n=10),the prednisone group(D,n=10) and the benazepri group(E,n=10) and each group was treated respectively.In each group renal tissue samples were collected at the 3rd week and the 7th week,respectively.The glomerular ultrastructure was examined by electron microscopy.Results:(1) After the adriamycin injection,a significant increase of the 24-hour urinary protein was observed at the 3rd week (P0.01).In AN rats,serum albumin level decreased (P0.01) while serum TC and TG levels increased (P0.01).(2)It was observed the diffuse fusion and effacement of foot processes and decrease of anionic sites in GBM in AN rats at the 3rd week.(3)At 7th week,the foot process counts of GBM dramatically increased in the C and D groups (P0.01),compared with that in the B group.Conclusion:The abnormal variation of glomerular ultrastructure was the important mechanism that lead to the occurrence and development of proteinuria in AN rats.It was possible that yuebi decoction had effects on nephrotic syndrome through improving glomerular ultrastructure in glomerulus.
