癫痫对儿童注意缺陷多动障碍发生的影响

摘　　要：目的观察儿童癫痫患者共患注意缺陷多动障碍(ADHD)的现状及其亚型特征;探讨癫痫自身的多种因素与发病的相关性。方法采用美国《精神疾病诊断与统计手册》第4版(DSM-Ⅳ)标准编制的儿童ADHD临床诊断性会谈量表对145例癫痫儿童进行评估,并与84例对照组进行比较分析。结果 145例癫痫儿童ADHD的检出率为46.89%,明显高于正常对照组20.24%。癫痫儿童ADHD主要表现为注意缺陷型;除苯巴比妥外未发现其他各种常用抗癫痫药物与ADHD的发生有关。回归分析服药治疗的癫痫儿童共患ADHD的可能相关因素,显示癫痫控制与否与其共患ADHD明显相关;回归分析首次发作的癫痫儿童共患ADHD的可能相关因素,未发现所列的因素对ADHD的发生有影响。结论癫痫儿童共患ADHD的风险较正常儿童高,以注意缺陷亚型常见;癫痫控制与否与其ADHD的发生明显相关。Objective To investigate the ratio of attention-deficit hyperactivity disorder（ADHD） occurred in children with epilepsy and the features of subtype in ADHD.To explore the association with the factors of epilepsy itself and the pathogenesis of ADHD.Methods All the enrolled 145 epilepsy children were underwent the test of ADHD which diagnosised with DSM-Ⅳ,then compared and analyzed with the control group.Results The ratio of ADHD in children with epilepsy was 46.89%,was significant higher than the normal control group which was 20.24% and it′s main subtype was attention deficit-type;It was not yet found the effect of the commonly used AEDS to the occurrence of ADHD in epilepsy children except Phenobarbital.The status of seizure control was the significant factor influencing the ADHD occurening in epilepsy children with AEDS;Regression analysis on the first onset of epilepsy children,the result was no evidence of factors listed infacted the ADHD occurrence.Conclusion The risk of ADHD occurring in children with epilepsy is higher than normal children,and the main features of subtypes is attention deficit-type;whether epilepsy is controlled or not is an important factor in ADHD occurrence
