摘 要:目的通过对大前庭水管综合征(LVAS)患儿听觉多频稳态诱发反应(ASSR)测试分析,探讨其ASSR特点。方法对40例(80耳)听性脑干反应(ABR)最大刺激强度95dBnHL未引出波形极重度感音神经性耳聋患儿进行ASSR测试,其中20例(40耳)为LVAS患儿,另20例(40耳)非LVAS患儿作为对照组。将两组5个频率ASSR的引出率、平均反应阈进行比较。结果在0.25、0.5kHz,LVAS患儿听觉多频稳态诱发反应引出率明显高于非前庭导水管扩大患儿,2个频率引出率差异有统计学意义(P<0.01)。在0.5、1kHz,LVAS与非LVAS患儿平均反应阈强度差异有统计学意义(P<0.05)。结论 LVAS多频稳态反应的特点是低频残余听力,对0.5、1kHz语言频段刺激较敏感。Objective To analyze and explore the characteristic of auditory steady-state response(ASSR)in infants and young children with larged vestibular aquaduct syndrome(LVAS).Methods The measurements of ASSR and brain stem response(ABR)were obtained from 40 infants(80 ears)with profound sensorineural hearing loss.No measurable responses of wave V in ABR was induced in all infants in this study,but acquired from the measurements of ASSR.Experiment group were twenty cases(40 ears)with LAVS,control group were twenty cases with non-LVAS.Compared with the positive incidence and the averaged response thresholds of ASSR in five different frequencies between LVAS patients and non-LVAS patients.Results The positive incidences of ASSR in infants and young children with LVAS were substantially higher than non-LVAS group at 250 Hz and 500 Hz tone stimuli.The difference of incidences between 250 Hz and 500 Hz tone had statistically significant(P〈0.01).At 500 Hz and 1 000 Hz ,the difference of averaged response threshold intensities between LVAS and non-LVAS group were also statistically significant(P〈0.05).Conclusion Characteristic of ASSR in the LAVS patient is the residual hearing in low frequency tone stimuli,the LVAS patients are sensetive to the tone stimuli at 500 Hz and 1 000 Hz.
