佩尔地平和硝普钠治疗高血压急症和亚急症临床对比研究

摘　　要：目的探讨佩尔地平和硝普钠治疗高血压急症和亚急症的疗效、安全性、适应证及不良反应情况。方法硝普钠组55例在常规治疗基础上给予硝普钠以0.5～8μg/(kg·min)静滴;佩尔地平组54例给予佩尔地平0.5～5μg/(kg·min)。观察用药前及用药后不同时间点血压、心率、伴随症状的变化及不良反应发生情况;同时记录用药前后血、尿常规、肝肾功情况。结果两组基线特征无显著差异,经降压治疗后,硝普钠组和佩尔地平组血压均显著下降,两组总有效率分别为96.4%和98.1%,差异无显著性(P>0.05);治疗前后两组心率均无显著增快;两组血、尿常规、肝肾功在治疗前后均无显著变化;硝普钠组和佩尔地平组不良反应发生率分别为7.3%和5.6%(P>0.05)。结论在适应证范围内应用硝普钠和佩尔地平治疗高血压急症和亚急症同样有效,二者降压的有效性和安全性没有显著差异,但二者适用范围有所不同,临床医师应根据其药理特点及适应证慎重选择才能实现更合理地用药。Objective To investigate therapeutic effect, safety, indication and adverse effects in treatment for Chinese patients with hypertension emergency with Perdipine or Sodium Nitroprusside. Methods Selective patients with hypertension emergency were divided randomly into Sodium Nitroprusside group （ n = 55 ） and Perdipine group （ n = 54） ： Nitroprusside group were given Sodium Nitroprusside at 0.5 -8μg/（ kg · min） ,0.5 - 5 μg/（ kg ·min） with intravenous injection on the basis of conventional therapy;Perdipine group were given Perdipine at 0.5 -5 μg/（ kg · min） ,0.5 -5μg/（ kg · min） with intravenous injection;then blood pressure, heart rate, simultaneous phenomenon and adverse effects at the different timepoints before and after treatment were investigated, and blood routine, urine routine, liver function, renal function before and after treatment were recorded. Results the differences in baseline characteristics between two groups wash＇ t significant, and blood pressures in both groups descend obviously after treatment and total effective power were respectively 96.4% and 98.1% in Sodium Nitroprusside group or Perdipine group（P〉0.05） ;and heart rate in both groups did not increase significantly after treatment; and the blood routine, urine routine, liver function, renal function in both groups did not change significantly after treatment. The incidence of adverse effects in two groups was similar （7.3% and 5.6% respectively, P 〉0.05）. Conclusion Effectivity and safety of treatment for chinese patients with hypertension emergency with Perdipine or Sodium Nitroprusside is similar if the drugs were used consistent with indication, but applicability of the two drugs is not same, and physician should select the suitable one according to their indications, therefore, the patients would get benefits to the most extent .
