血清C-反应蛋白对急性肺栓塞近期预后评估的初步研究

摘　　要：目的:探讨血清C-反应蛋白(C-reactive protein,CRP)水平对急性肺栓塞近期预后的评估价值。方法:51例急性肺栓塞患者通过螺旋CT肺动脉造影确诊,入院后均行血清CRP、血浆肌钙蛋白I、动脉血气分析以及经胸超声心动图等检查。根据血清CRP测定值分为CRP≥10mg·L-1组(n=21)及CRP<10mg·L-1组(n=30)。结果:CRP≥10mg·L-1组中低血压9例,右心室功能不全16例,心肌损伤13例,低氧血症12例;高危9例,中危10例,低危2例;入住ICU11例,3个月内死亡6例。CRP<10mg·L-1组中低血压4例,右心室功能不全14例,心肌损伤8例,低氧血症11例;高危4例,中危14例,低危12例;入住ICU5例,3个月内死亡2例。两组比较,低血压、右心室功能不全以及心肌损伤的发生率之间差异有统计学意义,P<0.05;在高危、低危、入住ICU人数以及死亡人数方面相比差异亦具有统计学意义,P<0.05。结论:血清CRP可作为急性肺栓塞患者的预后指标,协助评估病情、判断预后。Objective：To assess the value of serum C-reactive protein（CRP） for prognosis of patients with acute pulmonary embolism（PE）.Methods：There were 51 patients with acute PE,confirmed by computed tomographic pulmonary arteriography（CTPA）.Serum CRP,troponin I,arterial blood gases and echocardiography were measured.These patients were divided into two groups：CRP≥10 mg·L-1 group（n =21） and CRP 10 mg·L-1 group（n =30）.Results：In the CRP≥10 mg·L-1 group,9 had hypotension,16 showed right heart dysfunction,13 suffered myocardial damage,12 showed hypoxemia.Among these 21 cases there were 9 high-risk PE,10 middle-risk PE,2 low-risk PE,11 were admission of ICU and 6 died within 3 months.In the CRP 10 mg·L-1 group,4 had hypotension,14 showed right heart dysfunction,8 suffered myocardial damage,11 showed hypoxemia.Among these 30 cases there were 4 high-risk PE,14 middle-risk PE,12 low-risk PE,5 were admission of ICU and 2 died within 3 months.There was statistical significance in the occurrence of hypotension,right heart dysfunction and myocardial damage be-tween the two groups,P 0.05.Also there was statistical significance in the cases with high-risk,low-risk,admission of ICU and death,P 0.05.Conclusion：CRP can be used as a prognostic indicator for pulmonary embolism and can be used to assess the condition and prognosis.
