多西他赛联合顺铂、氟尿嘧啶治疗晚期胃癌的临床观察

摘　　要：目的:观察多西他赛联合顺铂、氟尿嘧啶治疗晚期胃癌的临床疗效及不良反应。方法:68例晚期胃癌患者采用多西他赛75mg·(m2)-1静脉滴注,第1天,化疗前预处理,顺铂20mg·(m2)-1,静脉滴注,第1～5天,氟尿嘧啶500mg·(m2)-1,静脉滴注,第1～5天,21天为1个周期,治疗至少2周期评定疗效。结果:完全缓解(CR)4例(5.9%),部分缓解(PR)29例(42.6%),病情稳定(SD)9例(27.9%),病情进展(PD)16例(23.5%),总有效率(CR+PR)为48.5%,临床受益率69.1%,中位生存期8.1个月,其中初治组有效率为54.2%(26/48),复治组有效率为35%(7/20),无CR病例,两组无显著差异(P>0.05)。不良反应为胃肠道反应、骨髓抑制及脱发。其中Ⅲ～Ⅳ度粒细胞计数减少占14.7%,Ⅲ～Ⅳ度恶心呕吐占8.8%,脱发100%,大部分患者可以耐受。结论:多西他赛联合顺铂、氟尿嘧啶治疗晚期胃癌疗效高,耐受性好。OBJECTIVE： To evaluate the effects and toxicities of the combination of docetaxel plus cisplatin,5 - fluorouracil in the treatment of patients with advanced gastric cancer. METHODS： 68 patients with advanced gastric cancer with docetaxel 75 mg · （ m2 ） -1 intravenously on day 1,pretreatment before chemotherapy,cisplatin 20 mg·（m2） -1,intravenous infusion of 1 ～ 5 days,5 - fluorouracil 500 mg·（m2） -1,intravenous drip note the first 1 ～ 5 days,21 day cycle,treat at least 2 cycles evaluated. RESULTS： The complete remission （ CR） 4 cases（5. 9% ）, partial remission（PR） 29 cases（42. 6% ）,stable disease（SD） 9 cases（27. 9% ）,disease progression（ PD） 16 cases （23. 5% ）,the total effective rate（CR ＋ PR） is 48. 5% ,Clinical benefit rate of 69. 1% ,the median survival of 8. 1 months,the effective rate of initial treatment cases is 54. 2% （26 /48）,and the retreatment cases is 35% （7 /20）,no CR cases,there is no significant difference in the two groups （ P〈0. 05 ）. Adverse events were gastrointestinal reactions,myelosuppression and alopecia. One Ⅲ ～ Ⅳ neutropenia accounted for 14. 7% ,Ⅲ ～ Ⅳ nausea and vomiting 8. 8% ,alopecia 100% ,most patients can be tolerated. CONCLUSION： Docetaxel combined with cisplatin, 5 - fluorouracil in the treatment of advanced gastric cancer with high and well tolerated.
