颅脑超声在糖尿病母亲新生儿脑发育方面的研究

摘  要:目的:探讨颅脑超声在糖尿病母亲新生儿脑发育方面的作用,为临床治疗及早期干预提供依据。方法:通过颅脑超声测量65例足月糖尿病母亲新生儿额叶脑回宽度,与148例足月非糖尿病母亲新生儿相比较,并分析母亲糖尿病控制情况与新生儿脑室变化。结果:糖尿病母亲新生儿额上回宽度(1.006±0.102)cm及其起始段脑回宽度(1.108±0.124)cm与非糖尿病母亲新生儿额上回宽度(0.859±0.087)cm,起始段脑回宽度(0.956±0.132)cm比较,差异有显著性(P<0.05)。母亲孕期糖尿病未控制组新生儿后角比值异常率(43.3%)高于控制组(20%),差异有显著性(P<0.05),受母亲糖尿病影响的新生儿脑发育过程中出现问题几率较高。结论:母亲孕期患糖尿病有可能造成子代脑成熟障碍,可通过颅脑超声对新生儿脑回宽度及脑室宽度测量予以评价。Objective:To explore the brain development in infants of diabetic mothers by brain ultrasound in order to provide a basis for clinical treatment and early intervention.Methods:Cerebral gyri width of lobus frontals was measured in 65 infants of pregnant diabetic mother by ultrasound and the parameters were compared with 148 infants of non-diabetic mother.We analyzed the relationship between the variation of gyri and ventricle width and the diabetic control of the mothers.Brain development status in neonates with diabetic mothers was observed.Results: Gyri width was significantly wider in the full term infants of diabetic mother(1.006 ±0.102) cm than that of the infants of non-diabetic mother(0.859 ±0.087) cm.This was more obvious in the gyrus frontals superior and initial section [(1.108 ±0.124) cm vs(0.956 ±0.132) cm](P〈0.05).The rate of ventricle posterior horn of un-controlled maternal diabetes(43.3 %) was higher than that of control groups(20%,P〈0.05).Conclusions: Maternal diabetes in pregnancy may result in filial generation brain immaturity.Cerebral ultrasound can be used to measure the cerebral gyri width and the ventricle width.
