躯体化障碍患者社会支持及应对方式特征的对照研究

摘　　要：目的 探讨躯体化障碍患者社会支持及应对方式的特征及其相关性.方法 42例躯体化障碍患者和42例正常对照者进行症状自评量表（symptoms checklist-90, SCL-90）、社会支持量表（Social Supporting Rating Scale, SSRS）和特质应对方式问卷（Trait Coping Style Questionnaire, TCSQ）测评,将所得结果进行分析.结果 （1）躯体化障碍组的SCL-90除敌对因子分外,总均分和其他8个因子分均明显高于正常对照组（P〈0.05）;（2）与正常对照组相比,躯体化障碍组的社会支持总分、主观支持分显著降低（P〈0.01）.特质应对方式的比较,躯体化障碍组的消极应对因子分高于正常对照组,积极应对因子分低于正常对照组,差异均有统计学意义（P〈0.001）;（3）躯体化障碍患者的社会支持总分、主观支持与SCL-90总分和各因子分（除焦虑、恐惧和偏执因子外）呈负相关,客观支持分与强迫、敏感和偏执因子分呈负相关（P〈0.05或P〈0.01）;消极应对分与SCL-90总均分及各因子分均呈正相关（P〈0.05或P〈0.01）.结论 躯体化障碍患者缺乏社会支持,且具有不良的应对方式,存在明显的心理问题.Objective To explore the characteristics of social supporting and coping style on mental status in patients with somatization disorder. Methods 42 somatization disorder patients and 42 normal controllers were evaluated with symptoms checklist -90（ SCL -90） ,social supporting rating scale（SSRS） ,trait coping style questionnaire（TCSQ）, the results were analyzed with t -test and correlation analysis. Results （ 1 ） All the factor＇s scores of SCL - 90 except hostility in somatization disorder patients were higher than those in the control group （ P 〈 0. 01 ）. （2） Compared with the normal controllers, somatization disorder patients had lower total and subjective social supporting scores, and they took more negative coping style, the differences were significant （P 〈 0. 05 ）. （3） The patients＇ total and subjective social Supporting scores were negative correlation with the scores of total and all factors except anxiety,fear and paranoid of SCL- 90, their objective social supporting score were negative correlation with the scores of obsess - compelled, sensitivity and paranoid of SCL - 90. The negative coping style were positive correlation with the scores of total and all factors of SCL - 90 （ P 〈 0. 05 ）. Conclusion Patients with somatization disorder lack social supporting and have negative coping style.
