摘 要:目的观察地塞米松、低分子右旋糖苷、地塞米松+低分子右旋糖苷对急性呼吸窘迫综合征(ARDS)模型兔血清IL—17、IL-23表达的影响。方法50只日本大耳兔随机分为空白组、模型组、地塞米松(地米)组、低分子右旋糖苷(低右)组、地米+低右组,每组10只。除空白组外,各组均制成ARDS模型,然后分别给予对应药物干预;干预后行血气分析,检测血清中IL-17、IL-23含量。结果模型组、地米组、低右组、地米+低右组血氧分压均显著低于空白组,血清IL—17、IL-23浓度高于空白组;地米组、地米+低右组血氧分压均高于模型组,血清IL-17、IL-23浓度均低于模型组;低右组血氧分压略高于模型组,差异无显著性;地米组、低右组、地米+低右组三组比较,地米+低右组血氧分压高于地米组,血清IL-17、IL-23浓度低于地米组;地米组血氧分压高于低右组,血清IL-17、IL-23浓度低于低右组。结论单纯低分子右旋糖苷治疗ARDS兔模型无显著性效果,地米+低右治疗ARDS兔模型优于单纯地塞米松治疗效果,地塞米松和低分子右旋糖苷合用可能有协同作用。Objective To observe the serum level of IL - 17, IL -23 in ARDS rabbit model, evaluate the intervention effect of dexamethasone plus dextran - 40. Methods 50 rabbits were randomly divided into 5 groups : control group, model group, dexamethasone group, dextran - 40 - group, dexamethasone plus dextran - 40 group, each group with 10 animals. Model group and dexamethasone group, dextran -40 group, dexamethasone plus dextran -40 group were injected with oleic acid by femoral vein to induce the ARDS rabbit model. Dexamethasone group, dextran -40 group, dexamethasone plus dextran - 40 group were given the intervention of dexamethasone, dextran - 40, dexametha- sone plus dextran -40 respectively. The blood was draw from femoral artery to detect the oxygen partial pressure and serum levels of IL - 17,IL - 23. Results The oxygen partial pressure of the model group were significantly lower in the model group,dexamethasone group,dextran-40- group,dexamethasone plus dextran- 40 group than the control group, but the serum level of IL - 17, IL - 23 was higher than the control group ; The dexamethasone group and dexam- ethasone plus dextran -40 group were higher than the model group in the blood oxygen partial pressure and the serum level of IL- 17 ,IL- 23; the dextran -40 group has a slightly higher than model group in the blood oxygen partial pressure,but the difference was not statistically significant; Compare among the dexamethasone group, dextran- 40 group and dexamethasone plus dextran -40 group, the blood oxygen partial pressure of dexamethasone plus dextran - 40 group was higher than dexamethasone group, the serum levels of L - 17, IL -23 was lower in the dexamethasone plus dextran -40group than the dexamethasone group; the blood oxygen partial pressure of the dexamethasone group was higher than the dextran -40 group, the serum levels of L - 17, IL -23 in the dexamethasone group was lower than the dextran - 40 group. Conclusion Dextran - 40 had no effect in treatment of ARDS rabbit model, but dexametha- sone plus dex
