血浆置换联合大黄灌肠对重型肝炎患者凝血功能的影响

摘　　要：目的观察血浆置换联合大黄灌肠对重型肝炎患者凝血功能的影响。方法将106例重型肝炎患者按抽签法随机分为治疗组54例和对照组52例,两组均给予综合治疗及血浆置换治疗,治疗组联合大黄液灌肠治疗。观察两组治疗前后血浆凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、血浆凝血酶时间(TT)、凝血酶原活动度(PTA)及血小板(PLT)的变化情况。结果治疗后两组PT、APTT、TT、PTA及PLT均明显改善(P<0.05),且治疗组各项指标改善优于对照组(P<0.05),总有效率治疗组81.5%,对照组57.5%,两组比较差异有统计学意义(P<0.05)。结论血浆置换联合大黄灌肠能有效改善重型肝炎患者的凝血功能,提高抢救成功率。Objective To observe the effect of plasma exchange（PE） combined with rhubarb enema on the coagulation function in patients with severe hepatitis.Methods 106 cases patients with severe hepatitis were divided into two groups：treatment group（n = 54） and control group（n = 52）,all the patients were treated with common medical treatment and plasma exchange,and on that basis the treatment group added rhubarb enema.The changes of PT,APTT,TT,PTA and PLT before and after treatment were observed.Results All indexes of two groups were improved significantly after treatment（P〈0.05）,which of treatment group were improved better than those of control group（P〈0.05）;the total efficiency of treatment group was 81.5%,while in control group was 57.5%,the two groups were significant difference（z = 2.214,P = 0.034） .Conclusion Plasma exchange combined with rhubarb enema could effectively improve the coagulation function and the success rate of rescue of patients with severe hepatitis.
