摘  要:目的:研究血管内皮生长因子(VEGF)与羊水量的关系,探讨羊水量调节的机制。方法:运用免疫组织化学法分别检测羊水过少的孕妇20例(羊水过少组)、羊水正常的孕妇22例(羊水正常组)、羊水过多的孕妇17例(羊水过多组)的胎盘、胎膜组织中VEGF的表达,半定量测其表达强度;同时每组按随机数字法抽取10例用免疫印迹法分别检测胎盘、胎膜VEGF的蛋白表达量。结果:①免疫组织化学:VEGF在3组胎盘、胎膜中均有阳性表达,其表达于羊膜上皮细胞、绒毛膜及胎盘滋养细胞的胞浆和小血管中,3组胎盘组织中VEGF的表达差异无统计学意义(P〉0.05),VEGF在3组胎膜组织中的表达强度随羊水量的增加逐渐增高,差异有高度统计学意义(P〈0.01)。②免疫印迹法:3组胎盘、胎膜组织中均有VEGF蛋白表达,胎盘中VEGF的蛋白表达量比较,差异无统计学意义(P〉0.05);VEGF在3组胎膜组织中IOD值随羊水量的增加逐渐增高,差异有高度统计学意义(P〈0.01)。结论:VEGF与羊水量的调节有关,随着羊水量的增加胎膜组织中VEGF的表达量有明显增高,VEGF在胎膜的上调可能促进了羊水的跨膜转运。Objective:To explore the relationship of vascular endothelial growth factor(VEGF) and amni- otic fluid volume (AFV), and possible regulation mechanism of AFV, Methods:The expression and its inten- sity of VEGF in human placenta and fetal membrane were measured by immunohistochemistry (IHC) in 20 cases of oligohydramniosis,22 cases of normal pregnancy and 17 cases of polyhydramniosis. 10 cases in ev- ery group were randomized selected to test VEGF protein expression in placenta and fetal membrane by western blot (WB). Results:①The expressions of VEGF were positive in placenta and fetal membrane of all three groups by IHC, the localization of VEGF was at capillaries and cytoplasma of epithelial cells of the am- nion membrane, chorion membrane, syncytiotrophoblasts and cytotrophoblasts of placenta. The expressions of VEGF in placenta were no difference among three groups ( P 〉 0.05). However, the intensity of VEGF expression increased significantly along with the increase of AFV among three groups( P 〈0.01 ). ②The ex- pressons of VEGF protein were positive in placenta and fetal membrane in all three groups by WB. And no significant differences in placenta were detected ( P 〉 0.05). Also VEGF IOD values increased significantly a- long with the AFV (P〈O. 01 ). Conclusions: VEGF has relationship with the regulation of AFV, the in- creased expression of VEGF accompanies with the increase of AFV. The up-regulations of VEGF protein ex- pression on the fetal embrane may mediate the increase of the transmembrane amniotic fluid transportation.
