摘  要:目的比较外周血单个核细胞(PBMCs)有或无HBV感染证据组中免疫功能的差异。方法常规分离78例CHB患者和10例健康对照组全血中的PBMCs,提取DNA,应用实时荧光定量聚合酶链反应(PCR)法检测PBMCs中的HBV DNA和HBV cccDNA;进行PBMCs的培养,孵育72h后,收集PBMCs上清液,采用酶联免疫吸附法(ELISA)检测干扰素-γ(IFN-γ)和白细胞介素-10(IL-10),并用四甲基偶氮唑盐(MTT)法检测PBMCs的增殖活性。结果PBMCs中HBV cccDNA阳性的患者组,其PBMCs产生IFNγ水平显著下降,IL-10水平显著升高,同时伴PBMCs的增殖能力显著下降。结论HBV感染PBMCs可能是HBV感染后机体Th1/Th2细胞因子比例失衡,特异性免疫功能低下,形成慢性HBV感染的主要原因之一。Objective To compare the immune functions of PBMCs in chronic hepatitis B(CHB)patients with and without HBV cccDNA in PBMCs.Methods After PBMCs were routinely isolated from whole blood samples and DNA was extracted from PBMCs,HBV DNA and HBV cccDNA were detected by real-time fluorescent quantitation polymerase chain reaction,respectively.Meanwhile,after PBMCs were cultured for 72 h,the supernatant were collected to detect the levels of IFNγ and IL-10 by ELISA,respectively,and the PBMCs proliferation was determined by MTT method.Results HBV cccDNA has been found in PBMCs of patients with chronic hepatitis B.There were lower PBMCs proliferation and imbalance of Th1/Th2 cytokines,indicating down-regulation of IFNγ and up-regulation of IL-10 expression in the chronic hepatitis B with HBV cccDNA in PBMCs than that in the patients without HBV cccDNA in PBMCs.Conclusions Therefore,PBMCs infected by HBV in chronic hepatitis B patients may be one of important factors responsible for the imbalance of Th1/Th2 cytokines,specific immunodepression against HBV and resulting in the persistent HBV infection.
